Heart sound analysis can provide an early clue to conditions associated with intracardiac thrombus formation.

Atrial fibrillation, characterized by an irregularly irregular rhythm, is an important risk factor for thrombus formation, particularly in the left atrial appendage.

Mitral stenosis, characterized by a low-frequency diastolic murmur and often an accentuated S1, increases left atrial pressure and impairs left atrial emptying. The resulting blood stasis contributes to the stasis component of Virchow's triad, particularly when atrial fibrillation is also present.
Heart failure: an S3 gallop or pulmonary crackles may indicate elevated filling pressures and cardiac dysfunction. In patients with severe LV systolic dysfunction, particularly with apical akinesis or dyskinesis following a large anterior myocardial infarction, the risk of left ventricular thrombus increases.

Cardiomyopathy-related murmurs and abnormal heart sounds may provide clinical clues to structural or functional cardiac conditions associated with thromboembolic risk, potentially before an embolic event occurs.

Knowing where to listen for aortic valve sounds can make a big difference in your cardiac assessment.

Aortic valve sounds and murmurs often radiate from the right upper sternal border toward the cardiac apex. The murmur of aortic stenosis is typically heard best in the aortic area, but it may also be audible at the apex and even radiate into the neck due to sound transmission through surrounding structures.

Heart failure and atrial fibrillation have long been treated as two separate conditions that frequently influence each other. New research suggests they may be more deeply interconnected, sharing genetic and molecular pathways, including changes involving the transcription factor TBX5 and coordinated shifts in the expression of more than 100 other transcription factors across atrial cardiomyocytes and fibroblasts.

This does not mean the two conditions are the same disease. Instead, it suggests that they may share overlapping biological mechanisms, opening new directions for understanding disease progression and developing future therapeutic approaches.

The ejection (systolic) murmur is one of the most characteristic acoustic findings in cardiology, arising from turbulent blood flow through a narrowed valve opening during ventricular ejection. On a spectrogram it appears as a high-frequency murmur with a distinctive diamond-shaped, crescendo-decrescendo pattern: the intensity gradually rises, peaks roughly at mid-systole, and then progressively fades.

This murmur begins shortly after the first heart sound (S1) and typically occupies the first two-thirds of systole, after which there is a brief period of silence — a pause — before the second heart sound (S2). This pause preceding S2 is a key acoustic clue that helps distinguish an ejection murmur from other systolic murmurs, such as a pansystolic (holosystolic) murmur.

The classic condition associated with this acoustic pattern is aortic stenosis, where narrowing of the aortic valve obstructs blood flow out of the left ventricle and produces this characteristic turbulence.
