I wanted to share that I have relocated to Cornwall to be closer to family. I truly enjoyed my time at FORM Osteopathy in Stratford and will greatly miss the amazing patients and colleagues I had the pleasure of working with.

As I settle back into my hometown, I’m excited to soon announce a new location in Cornwall Ontario. Stay tuned!

Lindsay is moving on from FORM to begin a new chapter closer to home in Cornwall!
We’re so grateful for all she has brought to our team and her patients. She will be missed, but we’re so excited for what’s ahead for her! 💛

We’re so excited to welcome Sarah Krijnen to FORM Osteopathy as our newest Osteopathic Manual Practitioner (OMP)! 🎉

Sarah is a recent graduate of the Canadian Academy of Osteopathy (CAO) and is passionate about helping patients move better, feel better, and support their body's natural ability to heal through gentle, hands-on osteopathic care.

She'll be seeing patients:
-Wednesdays: 2:00 PM – 6:00 PM
-Fridays: 9:00 AM – 1:00 PM
