Six months into FHO+, it looks less like a billing change and more like a practice-management change.

Hourly codes only pay if the time is captured. Continuity of care is a trend to watch, not an automatic one-quarter clawback. Outside use no longer hits the old access bonus, but it still shows up in continuity.
