What does it take to build a true system of care?

Join Ontario Health Teams, Primary Care Networks, Patient, Family and Caregiver Advisors, and health and social sector partners on November 25-26, 2026 for the Integrated Care Action Summit.

This year’s theme, Building a System of Care, explores how we can strengthen partnerships, align efforts across organizations, and improve outcomes for the people and communities we serve.

On June 8, the Western York Region OHT was pleased to be present at the Living Well Fair hosted at the Civic Centre Resource Library in Vaughan as part of Seniors Month.

Events like these highlight the importance of meeting people where they are, listening to their experiences and ensuring they know how to access the care and supports available in their community.

As the Western York Region Ontario Health Team OHT, we were pleased to share information on:
• How residents can connect with a family physician or primary care team.
• The role of the Western York Region OHT in improving healthcare access and coordination across Vaughan, Richmond Hill and King Township.
• Community supports and resources available to help residents navigate the healthcare system with greater ease.

The conversations we had throughout the day reinforced the value of community engagement and collaboration in building a more connected, patient-centred health system.

Thank you to Vaughan Public Libraries and everyone who stopped by our table to learn more, ask questions and share their experiences. We look forward to continuing our work to strengthen access to care across Western York Region.

Recently, we were thrilled to attend Winterfest in Vaughan as it’s a fantastic way for folks to get together and enjoy a wide range of family-friendly activities.

We had a wonderful time connecting with our community, hearing about their unique health care needs, and sharing how we are working to address those needs.

SCOPE is a great example of how OHT collaboration brings together hospital, primary care and community partners to improve how patients and providers connect to care across our region.

When it comes to recognizing the incredible contributions of our staff and physicians, Mackenzie Health’s Kudos Awards are a meaningful way to celebrate excellence. We’re proud to congratulate the SCOPE (Seamless Care Optimizing the Patient Experience) team on their recent Kudos Award win in the Innovation category.

This award recognizes the team’s work to improve how patients and providers connect to care in western York Region. Through SCOPE, family doctors and nurse practitioners in the community can access hospital-based services through a single point of contact, leading to faster support for patients, easier access to specialist advice for providers and fewer unnecessary emergency department visits.

In just over six months, the team reached a level of physician participation that typically takes years to achieve.

This team is part of the greater Western York Region Ontario Health Team, where Mackenzie Health and partner organizations work together to connect hospital, primary care and community services and support better care across the region.

Congratulations to the team on this well-deserved recognition! 🎉

Respiratory illnesses, like the flu and COVID-19, are more common during the fall and winter. Getting vaccinated helps reduce the spread of these viruses and prevents serious illness.

Starting Oct. 27, free flu and COVID-19 shots will be available to the general public at participating pharmacies, participating doctor and nurse practitioner offices and some public health units.
