Health Data Coalition The Health Data Coalition is a physician-led data sharing initiative funded by the GPSC. Are you looking to use existing primary care data in a meaningful way?

Access and use EMR data beyond your immediate practice to answer clinical questions? In a safe and secure environment? Managed by a trusted partner? The Health Data Coalition (HDC) – an amalgamation of the knowledge, assets, and experiences gained within the Physicians Data Collaborative (PDC) and AMCARE – is a physician-led data sharing initiative funded by the General Practice Services Committee

(GPSC). The HDC supports a learning health system and is
designed to aggregate patient level data from individual physician practices, across disparate EMRs, and use it as a feedback mechanism to support self-reflective practice for the individual physician member, as well as evaluation of PSP learning modules, small group peer learning, Division-led and provincial projects and BC Ministry of Health system planning and population metrics. We offer a flexible, accessible platform that provides HDC users access to a robust source of medical care data, while protecting provider and patient identity, and ensuring that physicians control the data sharing. More than a technical platform, the HDC is about and built on relationships. Together, we work with HDC users and partners to:

• Unleash the immense – and as yet untapped – potential of EMR data from community physician offices

• Connect and empower physicians to enhance patient care and contribute to a sustainable health care system through reflective personal and small group Quality Improvement

• Remain actively engaged in building and improving HDC through fair process, sustainable funding and transparent governance models.

COVID-19 has raised everyone’s awareness of the risk of clinically vulnerable patients developing a serious systemic disease that can result in significant morbidity and mortality. Chronic respiratory disease is a category of conditions, including COPD and asthma, where the risk of hospitalization and death is increased.
