✨ Almost 20 eggs. One healthy embryo. A pregnancy. Then a miscarriage with no clear explanation.

You were told you’re a poor responder after IVF.

But that describes what happened during one cycle. It doesn’t necessarily explain why it happened.

Before jumping into another protocol, there are bigger questions worth asking:

What happened during the 90 days leading up to stimulation?

Were there factors affecting follicle development before the injections even started?

And has anyone looked beyond changing the medication?

Because stimulation can only work with the follicles that are already there.

Before you do, there are 5 questions I’d want answered.

Because “it didn’t work” isn’t enough information.

Where did the cycle actually break down?
Was the protocol right for you?
What could be affecting egg or s***m quality?
Has implantation been fully investigated?
And most importantly what are we doing differently next time?

Your failed cycle contains information. The goal is to use it before making your next decision.

You don’t want to become “that patient.”

The one asking too many questions.

The one bringing a folder.

The one asking if anyone can explain why.

So instead, you tell yourself to trust the process.

You book another consult.

Another IVF cycle.

Another second opinion.

Or you hear the words “it’s probably time to consider donor eggs,” and wonder if that’s really the only path left.

But the question keeps coming back.

Did anyone actually finish looking?

Not:

* Did they adjust the medication?
* Did they change the protocol?
* Did they recommend another cycle?

But…

Did anyone explain why your body responded the way it did?

Were all the pieces actually investigated?

Your AMH was never the whole story.

An egg spends roughly 90 days maturing in constant conversation with your internal environment.

One marker I often discuss is hsCRP, a standard blood test that can provide clues about systemic inflammation. It’s not a fertility test, but it can help identify patterns that deserve further investigation as part of a comprehensive assessment.

No IVF stimulation protocol changes the environment your eggs developed in over those previous 90 days.

That’s why looking beyond AMH matters.

Depending on your history, other markers such as homocysteine, ANA and ferritin may also help complete the picture.

What wasn’t evaluated?

A donor egg recommendation may absolutely be the right path. But before making that decision, ask whether the recommendation was based on the full biological picture or just your AMH, FSH, and antral follicle count.

Those numbers tell you what’s happening today. They don’t explain why.

Questions worth asking include:

* Was your thyroid fully evaluated?
* Were autoimmune factors considered?
* Was your gut health or nutrient status assessed?
* Was your uterine environment evaluated?
* Was your partner’s s***m DNA integrity reviewed?

Sometimes donor eggs remain the best option.

Sometimes the evaluation changes the conversation.

Either way, you deserve to make that decision with the fullest picture possible.

Most people think IVF starts with stimulation.

It doesn’t.

By the time you begin injections, the eggs recruited for that cycle have already been developing for weeks to months.

Changing the protocol may change how many eggs you retrieve.

It doesn’t erase the metabolic, inflammatory, hormonal, or mitochondrial environment those eggs developed in.

And it’s not just about egg quality.

If his semen analysis is “normal” but no one has looked at DNA fragmentation, you’re still missing part of the picture.
