We’re proud to be partnering with Health Innovation Action and Health Innovation Group to bring the next Practice Facilitation Program to healthcare professionals across Canada.

Launching in October 2026, this national program is designed to build the confidence, knowledge and practical quality improvement (QI) skills needed to test, implement and sustain meaningful change in primary healthcare.

Participants will gain:
• Practical skills in practice facilitation and QI
• Coaching strategies to support primary healthcare teams
• Evidence-based tools and resources
• Peer-to-peer connections with practice facilitators
• Mentorship from health and social care transformation leaders

The program runs October 2026–February 2027 and is delivered virtually.

🌟 Board Member Spotlight: Gillian Sweeney

This month, we're spotlighting Gillian Sweeney, whose career has spanned clinical care, health care leadership, and digital health transformation.

Gillian’s professional journey began as a registered nurse in neonatal and pediatric care. After earning her MBA, she transitioned into strategic roles in health care before joining the Newfoundland and Labrador Centre for Health Information as Vice President, Clinical Information Programs and Change Leadership. In this role, she led the province’s digital health strategy, overseeing the provincial implementation of electronic health records, virtual care and change management frameworks.

In 2020, she was recognized as a Woman Leader in Digital Health by Digital Health Canada and was also awarded the Leadership in Digital Health Award by the Canadian Health Informatics Awards. Her current role is Assistant Deputy Minister with the Government of Newfoundland and Labrador.

Gillian is passionate about evidence-based, system level change that is grounded in stakeholder engagement. She currently serves on the Hockey Newfoundland and Labrador Board of Directors as Secretary and Chair, Governance Committee and has previously served on the Digital Health Canada Board of Directors.

Employee Spotlight: Sara Di Gregorio, Program Manager 🌟

Sara Di Gregorio is a Program Manager at Amplify Care, with demonstrated experience leading cross-functional teams and driving complex initiatives in the digital health space.

At Amplify Care, Sara supports the delivery of digital health programs across Ontario and other provinces, with a focus on enabling successful implementation and adoption. She works closely with healthcare organizations and change management specialists to help teams integrate new solutions and workflows into clinical practice.

Sara has contributed to the implementation of initiatives such as eReferral and virtual care, helping bring digital health solutions from planning to practice. Her collaborative approach and focus on change management help ensure that new technologies are not only implemented, but successfully adopted by the people and organizations using them.

Digital health works best when it’s built with clinicians, not simply for them.

At Amplify Care, co-design is a fundamental part of how we approach digital health. The people using a tool every day are best positioned to identify what works, what creates friction, and what could better support their practice.

Our latest blog explores how clinician feedback is helping shape enhancements to the E2P EMR-integrated diabetes tool for TELUS PS Suite, from more flexible documentation and streamlined medication management to localized referral pathways and an updated user experience.

Because integration into the EMR is only part of the solution. Digital health tools need to reflect the realities of care delivery.
