In endodontics, more is not always the objective.

Every clinical decision comes with a balance: creating the conditions for effective disinfection while preserving as much natural tooth structure as possible.

And that balance becomes especially relevant when we look at the complexity of the root canal system.

Because successful treatment is not simply about how much we shape. It is about how we combine preparation, irrigation and technology to address the anatomy we are actually treating.

Does more shaping really mean better cleaning?

Mechanical preparation is essential in root canal treatment — but anatomy does not always follow the path of the file.

Fins, isthmuses, recesses and other anatomical complexities may remain beyond direct instrument contact, even when canal preparation is increased.

This is one of the principles behind Root Preservation Therapy: instead of removing more tooth structure to make treatment reach further, we ask how debridement can extend beyond the areas directly contacted by files.

Retreatment means working with more than the anatomy of the tooth.

It also means working with the decisions, materials and limitations of the previous treatment.

In this case, a previously treated and crowned tooth presented with asymptomatic apical periodontitis and a large apical lesion.

One year after single-visit retreatment with OdneClean and BC sealer, the lesion showed a clear radiographic reduction and the PAI score changed from 4 to 2.

The lesion had not completely disappeared.

But meaningful healing does not always mean immediate or complete radiographic resolution.

It means assessing the direction in which the case is evolving and continuing to follow it over time.

Biofilm is not simply bacteria sitting on a surface.

It is a structured microbial community protected by its own matrix — making it harder to disrupt, especially in the complex anatomy of the root canal system.

Isthmuses, lateral canals, ramifications, dentinal tubules and apical complexities can all remain beyond the direct reach of instruments.

That is why effective cleaning goes beyond shaping.

It requires movement, activation and fluid dynamics capable of extending cleaning into areas that files cannot physically access.

The immediate post-operative radiograph is only the beginning of the story.

At that stage, the periapical lesion was still visible.

It was the one-year follow-up that provided a clearer picture of how the case had evolved.

This case by Dr. Jenny He is also particularly interesting because the treated tooth was an abutment of a bridge, adding another layer to the clinical decision-making process.

At the one-year assessment, no periapical lesion was radiographically detectable, and the PAI score had changed from 4 to 1.

A reminder that treatment outcomes cannot always be evaluated on the day the canals are filled.

They are revealed through time, follow-up and careful reassessment.

A canal can look clean and still hide what matters most.

That is one of the biggest challenges in endodontics.

Root canal systems are not simple, straight spaces. They contain anatomical complexities where microorganisms and biofilm can persist beyond the direct reach of mechanical instrumentation.

This is why visual cleanliness alone cannot tell us the full story.

Successful treatment requires us to think beyond what has been physically removed and consider what may still remain in areas we cannot see or directly touch.

Because in endodontics, looking clean and being disinfected are not the same thing.

And understanding that difference can change the way we approach treatment.
