A lower creatinine usually looks like better kidney function—but creatinine can also fall when someone loses muscle mass.

This video raises that concern in the context of the TEMPO trial used to approve tolvaptan for PKD, and asks why cystatin C data—another kidney-function marker that is less dependent on muscle mass—was not published.

When kidney function drops, it is easy to panic and start cutting protein, restricting salt, or waiting months without changing anything.

This video explains why an improved eGFR number does not automatically mean the kidneys recovered—and why PKD nutrition should focus on a tailored plan instead of random restrictions. Comment “PLAN” for the basic PKD meal structure.

If you have PKD, a normal GFR may not show the full picture.

Your kidneys have extra filtering capacity, so blood tests can remain in the normal range while cysts grow and functioning tissue is gradually lost. An MRI can show this progression by measuring kidney growth.

The takeaway: don’t wait for GFR to fall quickly before acting. Earlier action may give you more opportunity to protect the kidney tissue you still have.

A new PKD diagnosis can feel like an eclipse over your life: darker, frustrating and even hopeless. But an eclipse is not the sundown.

A theme park day doesn’t have to rely on whatever food is available inside. 🎢 For our family trip, I packed carrots, nuts, cheese blocks, a whey protein shake with collagen, monk fruit, fiber and MCT oil, two apples, and a big bottle of water.

The apples may raise the carb count a little, but with plenty of walking, that felt reasonable. Perishables went in a cooler bag; the rest went in a backpack. Simple, quick grocery-store food for a PKD/keto-style day on the road.

A study of more than 22,000 matched adults found that an individualized program using carbohydrate reduction, nutritional ketosis, moderate protein, regular monitoring, and ongoing coaching was associated with a 36% lower risk of developing chronic kidney disease and a 43% lower risk of reaching stage 3 CKD or worse.

The researchers also found no increased risk of kidney stones, metabolic acidosis, ketoacidosis, or gout.

You may be following your PKD plan, eating keto and doing everything “right”—but constant fight-or-flight can still affect sleep, cortisol, blood sugar, hormones and ketosis.

During an intensely stressful period, pushing the diet harder may add another burden. Sometimes the more important step is addressing the job, relationship or environment keeping you in survival mode, giving yourself some flexibility, and returning to the plan when you have more capacity.

Less salt isn’t always better—and the relationship between sodium, blood pressure, and cardiovascular risk is more nuanced than many people realize.

An analysis of more than 133,000 people across 49 countries found that higher sodium intake was associated with higher blood pressure, especially in people with hypertension. Yet the lowest observed cardiovascular risk was around 4–5 grams of sodium per day, or 10–12 grams of salt.

On a ketogenic diet, restricting salt too aggressively may also contribute to fatigue, dizziness, and dehydration. Salt your food to taste, and remember that blood pressure is only one part of the picture. 🧂

“Eat less protein. Avoid salt. Come back in six months.”

Kidney advice is often far more complicated than that. Protein intake can change creatinine results, and salt sensitivity may be connected to potassium status. Your blood tests and individual situation matter. 🩺🧂🥩
