Friction. Shear. Microclimate. These three forces are recognised in clinical guidelines as central to how and why pressure injuries develop¹, and they’re at the heart of what we’ll be discussing at EPUAP 2026 in Dublin.

Get any one of them wrong, and skin integrity is put at risk: friction as skin moves against a surface, shear as underlying tissue layers move against each other, and microclimate as heat and moisture build at the skin’s surface. Understanding how the three interact isn’t just theory. It shapes real decisions, from repositioning protocols to support surface and dressing selection.

While debridement is a critical part of wound bed preparation, achieving and maintaining a clean wound bed can be challenging when biofilm persists. ⁶,⁷
