مع احتمال وجود إصابة قلبية

الروشتة متنفعش لطفل عشان الأسبرين
نتجنبه مع الاطفال خوفاً من الإصابة بمتلازمة خطيرة ← Reye syndrome

حاليا توصف مضادات التهاب NSAIDs زي نابروكسين

تحديد بروتوكول العلاج يكون حسب الحالة

Welcome to the answer {Case × Prescription 🗒️}

The case we had was → Rheumatic fever
with possible cardiac involvement.

This prescription is not generally suitable for children because it contains aspirin; aspirin is usually avoided in children due to the risk of a rare but serious condition → Reye syndrome.

Nowadays, NSAIDs (non-steroidal anti-inflammatory drugs) such as naproxen may be used in some cases.

The treatment protocol is determined according to the patient’s condition and the doctor’s assessment.

الإجابة كانت3️⃣ سبب الكحة دواء تاني وهو دواء الضغط ACEi ودة السبب أنه مش بيستجيب لأدوية الكحة أو أن الكحة مش بتتحسن
وعشان كده
الحل هو تغيير نوع الدواء إلى ARB في الأغلب أو نوع تاني مناسب حسب تقييم الطبيب وحالة المريض

Active ingredient → Atorvastatin

Lipidnorm 10 mg
Active ingredient → Ezetimibe
Used to:
🔸 Lower elevated cholesterol levels in the blood
🔸 Reduce cholesterol absorption from the intestine
🔸 Help lower LDL-C levels
and consequently contribute to reducing the risk of cardiovascular disease and blood clots.

There is also Lipidnorm-Co, which contains:
Ezetimibe
&
Simvastatin
to reduce cholesterol absorption from the intestine and reduce cholesterol synthesis in the liver.

لأنه قد يسبب اضرار خطيرة للجنين 👶منها⬇️
♦️إنخفاض وظائف الكلى للجنين
♦️نقص السائل المحيط بالجنين Oligohydramnios
♦️عدم اكتمال نمو الرئة🫁
♦️تشوهات في عظام الجمجمة 💀

It is contraindicated during the second and third trimesters of pregnancy.
Why?! 🤔
Because it may cause serious fetal complications 👶 including:
♦️ Reduced fetal kidney function
♦️ Oligohydramnios (low amniotic fluid)
♦️ Impaired lung development 🫁
♦️ Skull bone abnormalities 💀

الاثر الجانبية: الإسهال diarrhea
بتحصل ليه؟!

بيقلل البكتيريا النافعة اللي بتحمي الأمعاء ←
تفقد الأمعاء قدرتها على مقاومة العدوى ←
زيادة↑ نمو بكتيريا C. difficile←
تنتج السموم بتاعتها toxin A & toxin B←
تسبب تلف والتهاب في بطانة القولون← اسهال diarrhea وقد تتطور إلى← التهاب القولون الغشائي الكاذب

Why does it happen?!

Clindamycin ↓
Reduces beneficial gut bacteria →
Loss of colonization resistance →
↑ Growth of C.difficile →
Toxin A & B production →
Colon inflammation and damage →
Diarrhea, which may progress to Pseudomembranous colitis.
