π₯ PDC Case of the Month | May 2026 β Gynaecology MRI
Uterine fibroids affect up to 70% of women by age 50 β but their true extent is often underestimated on ultrasound. This month's case illustrates why MRI is the definitive imaging modality for fibroid characterisation.
A 44-year-old female presented with heavy menstrual bleeding, significant abdominal distension, pelvic pressure, and urinary frequency. MRI of the pelvis revealed a giant intramural fundal fibroid measuring 25 Γ 15 Γ 21 cm, with extensive cystic degeneration β fluid-filled cavities indicating benign degenerative change. The uterine cavity was displaced inferiorly, and there was left-sided mild hydronephrosis from ureteric compression, a 6.2 cm left ovarian simple cyst, and displacement of both the bladder and re**um.
Diagnosis: Giant Intramural Uterine Fibroid with Cystic Degeneration
This case underscores the critical role of MRI in surgical planning. By precisely mapping the fibroid's location, size, vascularity, and degeneration type, our report gave the surgical team the confidence to plan the most appropriate intervention β and to anticipate the ureteric and bladder involvement before entering the operating theatre.
