Over a multi-week course of radiotherapy, a patient's anatomy doesn't stay still. Tumors shrink, weight changes, organs shift. Offline adaptive workflows account for this by remapping contours onto updated scans between fractions, but the standard method for doing it has real limits.

Cone-beam CT images are noisier than the original planning scan, and shrinking tumors can throw off the deformation models used to track anatomical change. Both problems have kept manual review a heavy, slice-by-slice requirement.
