TRUE: Isolated low ferritin is the earliest sign of low iron stores — it’s the most specific marker we have, so it can drop before serum iron or saturation ever look abnormal. So step one is always addressing intake and absorption.

BUT: When ferritin stays stubbornly low despite good iron intake, it’s worth looking at the whole chain, not just the supply.

Keep your eyes on these patterns:

→ Heme iron intake — adequate, and actually being absorbed?

→ Stomach acid — low HCL impairs how well iron is taken up.

→ Inflammation — ferritin rises with inflammation, so a “normal-ish” ferritin in an inflamed person may overstate true stores (worth pairing with hs-CRP, and considering hepcidin).

→ Infection or chronic immune activation — traps iron in macrophages and hepatocytes, blocking absorption.

→ Cofactors — biotin, copper and vitamin A support the ferroxidase step that loads iron for transport and storage

The point isn’t “take more iron.” It’s identifying which link in the chain is actually limiting — then addressing that.
