Do they… or were they simply overfilled, over-touched, or poorly executed? 👀

This is my client’s lips 3 years after her FIRST 1ml.
No migration.
No shelf.
No lumps.
No overfilled “duck” effect.

Just soft, feminine structure that still moves naturally when smiling, pouting, and at rest.

“Demonized” doesn’t always mean dangerous.
Sometimes it means misunderstood.

Tear trough filler is still one of my favourite treatments in aesthetic medicine when the patient is selected correctly, the product is chosen carefully, and the technique is performed with extreme precision.

This is not a “quick filler area.”
This is millimetre work.

The under-eye area is delicate, vascular, unforgiving, and technically demanding — which is exactly why I approach it with almost obsessive attention to detail. Those who have watched me work know… the room goes quiet, my breathing slows down, and for a moment it’s just me, the cannula, anatomy, and precision.

Because successful tear trough treatment is not about “filling a hollow.”
It’s about understanding:
• facial anatomy
• fat pad support
• ligament structure
• tissue quality
• light reflection
• product rheology
• and knowing when NOT to treat.

For this procedure, I work with Teosyal Redensity II — one of the very few fillers specifically designed and indicated for the delicate under-eye area. Its formula combines crosslinked and non-crosslinked hyaluronic acid together with amino acids, antioxidants, minerals, and vitamins, allowing it to behave almost like a hybrid between a filler and a skin quality treatment.

Which means the result you see immediately after treatment is not the final result.

Over the following weeks, the product continues to integrate beautifully within the tissue as hydration and skin quality improve. The goal is never puffiness or heaviness. The goal is for somebody to look:
• fresher
• healthier
• more rested
• softer
without anyone being able to tell why.

Tear trough filler restores deep structural support on the periosteum. But if superficial creasing, thin skin, or poor tissue quality still remains after swelling settles, this is where regenerative treatments such as polynucleotides can elevate the result even further.

It’s all about biomechanics. 💋

When I assess lips, I don’t only look at them at rest.
I analyse them in motion, under tension, during smiling, compression, speech, and muscular activation…👀 because lips are not static tissue. They are a dynamic structure constantly reacting to pressure, movement, and weight distribution.

These lips had:
• naturally tight tissue
• a slight M-shape pattern
• central heaviness under the Cupid’s bow
• lateral volume deficiency
• inward collapsing tendency during movement

And this is where inexperienced injecting often goes wrong.

Most injectors would instinctively place more filler directly into the centre to “build height”.
But anatomically, that would increase the weight load in the heaviest part of the lip.

What happens next?

The centre becomes overloaded.
The lateral columns lose support.
The lip starts curling inward during animation.
The sides collapse.
And the lips begin looking heavy instead of elevated.

So instead of chasing volume, I built structural support first.

The lateral lip body was treated almost like architectural scaffolding, creating support columns capable of holding and redistributing the natural tissue weight underneath the Cupid’s bow.

This allowed elevation without overfilling the centre.

That’s why the lips look:
• lifted instead of swollen
• structurally balanced instead of “done”
• soft in motion
• supported at rest
• naturally reflective under light without looking stiff

For this case, I selected Stylage M because the tissue required controlled structure and support. Tight tissue behaves very differently to soft, elastic tissue .. which is why advanced lip work is never “one filler fits all.”

The filler is only the tool.
The real work is understanding tissue mechanics.

This is what the future of aesthetic medicine looks like:
less copy-paste injecting, more anatomy, biomechanics, and intelligent treatment design.

I almost forgot…
this was the whole point.

Somewhere between the pressure, the burnout, the noise online, the endless opinions, the “posting is cringe” era, the negativity around aesthetics, university, real life, and trying to hold everything together…

…I lost the spark for a moment.

Not the skill.
Not the passion.
Just the connection to it.

And that scared me a little.

Because this industry has been my life for 11 years.

11 years of studying faces, skin, ageing, beauty, anatomy, technique, artistry, psychology, science.
11 years of building these hands.
11 years of becoming the practitioner I once dreamed of becoming.

I think burnout made me forget how far I’ve actually come.

But recently something shifted again.

I started remembering:
I genuinely love this world.

I love the science.
I love the artistry.
I love travelling to international congresses and learning from the best in the world.
I love regenerative medicine, longevity, innovation, and the future of aesthetics.

And honestly?

The fact that I now get to continue this journey while studying to become a healthcare professional in the UK feels surreal.

It’s a hard chapter.
Probably the hardest one yet.

But I know one day I’ll look back and realise this was the chapter that opened the biggest doors.

Maybe I didn’t lose the spark after all.

Maybe I was just exhausted,

And this is only the beginning. 💗💅💋🙆🏼‍♀️

“Just dissolve it” is not always the only answer.

A lot of my clients come to me after years of filler migration, uneven volume, lumps, poor placement, or lips that simply don’t move naturally anymore… but they are terrified of hyaluronidase. And honestly? I understand why.

Hyaluronidase is not a “magic eraser.”
It is a powerful enzyme designed to break down hyaluronic acid — and that includes not only filler, but potentially your own naturally occurring HA within the extracellular matrix.

When used aggressively, excessively, or repeatedly, it may contribute to:
• temporary tissue dehydration
• compromised skin quality
• increased laxity in delicate tissues
• inflammation and prolonged swelling
• disruption of the structural hydration that gives youthful skin its elasticity

Especially in lips that have already been through multiple rounds of filling and dissolving, the tissue can become fibrotic, inflamed, fragile, and unpredictable.

This is why advanced correction work is not about blindly adding more product OR dissolving everything instantly.
It’s about understanding:
— anatomy
— tissue behaviour
— filler rheology
— migration patterns
— scar tissue mechanics
— and knowing when the tissue can still be strategically reconstructed.

In selected cases, irregularities can be softened and visually corrected through precise placement, structural balancing, and controlled support — without overfilling the lips and without reaching immediately for large amounts of hyaluronidase.

Not every pair of lips is a candidate for this approach.
Some lips absolutely DO need dissolving first.
