Finding your way around CGH Partnership services – help from your Patient Participation Group
Not sure how to book an appointment, order a prescription, follow up a test result or referral, or know what to do when you haven’t received an expected response?
The CGHB Patient Participation Group (PPG) Health and Wellbeing Hub has a practical, easy-to-follow guide to help patients navigate CGH Partnership services.
The page brings useful information together in one place, including:
✅ Appointments and accessing medical help – understand the routes available and what should happen next.
✅ Prescriptions and medicines – find information about repeat prescriptions and contacting the Pharmacy Team.
✅ Test results and referrals – understand follow-up and what to do when an expected update hasn’t arrived.
✅ Your records and online services – find guidance on the NHS App, Evergreen Life and access for carers.
✅ Communication and accessibility – information about interpreters, large print, communication support and reasonable adjustments.
✅ Practical help and support – including home visits, registering with the practice, transport, complaints and local health and wellbeing services.
The guide also highlights areas where local processes need further clarification, helping patients understand what is published and where information is still awaited.
Why use it? The Hub is an independent, volunteer-led patient information resource designed to make finding the right information easier, help patients understand their next steps and support improvements in patient experience.
It does not book appointments or replace clinical advice, and it is not an official CGH Partnership page.
Take a look and share it with anyone who might find it useful:
