Bone health isn't always top of mind, especially when life gets busy. But looking after it now could make a big difference later.

If you're a woman, you're significantly more likely to be affected by osteoporosis. That's why simple habits like staying active, eating calcium-rich foods and maintaining healthy vitamin D levels are worth prioritising.

Find out more about how to stay bone healthy or find local support if you need it.

πŸ‘©β€βš•οΈ Do you know who is actually in your GP practice team?

πŸ‘‰ Find out here:
https://cghppg.weebly.com/primary-care-teams.html

Your GP practice is much more than just GPs.

You may be able to get help from a practice nurse, pharmacist, physiotherapist, mental health practitioner, social prescribing link worker, care coordinator or other members of the team.

But do you know who does what β€” and why you might be offered an appointment with them?

Knowing how the team works can help you:
βœ… get to the right person
βœ… understand your options
βœ… ask the right questions
βœ… navigate your GP practice with more confidence

This is one of a series of simple patient guides we’ve created to help people navigate primary care.

πŸ“– Take a look β€” it could save you time and help you get the right support.

πŸ“‹ TEST RESULTS, REFERRALS & β€œWHAT HAPPENS NEXT?”

Have you ever wondered:

❓ Has my test result actually been received?
❓ Has my referral been sent?
❓ Who is responsible for the next step?
❓ What should I do if I haven’t heard anything?

πŸ‘‰ We’ve created a new CGHB PPG FAQ to answer these questions β€” and many more.

πŸ“– Read the answers and practical guidance here:
πŸ‘‰ https://cghppg.weebly.com/results-and-referrals.html

Why have we created it?

The journey from GP appointment β†’ test β†’ result β†’ referral β†’ hospital appointment β†’ follow-up can be confusing.

It isn’t always obvious who is responsible for the next step, who to contact, or what you should do if something appears to have been missed.

Our new page brings together the questions patients commonly ask, with practical guidance and useful links to help you understand what happens next.

πŸ”Ž There may be an answer to a question you’ve been wondering about.

Please share this post with family, friends and carers who may find it useful.

CGHB Patient Participation Group
Helping patients navigate local NHS services.

🌐 **A clearer digital front door for CGH patients** www.cghppg.weebly.com/practice-info

The Patient Participation Group has rebuilt our **Practice Information** section of our website to show how finding help online could be made simpler and clearer.

Instead of patients having to search across different pages, the new hub starts with **what you need help with**. It brings together information and FAQ about:

* Making an appointment
* Ordering prescriptions
* Test results and referrals
* Blood tests and samples
* The different healthcare professionals who may help you
* Home visits and registering with the practice
* Accessing services for someone else
* Self-referral and other sources of support
* Making an effective complaint and seeking a practical resolution

The page is **live now** and can be used by patients. It is also a practical demonstration of the improved **β€œdigital front door”** the PPG is suggesting to the practice. It shows how clearer, patient-friendly information could work and could make a future rebuild of the practice website easier, improve efficiency and improve patient services.

We have also tried to be open about what is known: the pages distinguish between information published by CGH, what the NHS says should happen, and local processes that still need to be confirmed.

πŸ‘‰ **Explore the new (PPG) Practice Information hub:**

Please take a look and tell us: **Is it easy to find what you need? What could still be clearer?** Your feedback will help us demonstrate what works and suggest further improvements to the practice.
