I thought this might be arthritis… but it wasn’t.

She came in with joint pain that:
– built through the day
– improved overnight
– and followed a clear monthly pattern

Naturally, the concern is autoimmune disease.

But her bloods were normal.
No inflammatory markers.
Nothing to suggest classic arthritis.

What stood out was the pattern.

Her joints were hypermobile — so stability relied heavily on muscle support.
By the evening, those muscles were fatigued.

Then after ovulation, joint stability drops further —
and if energy intake is low, that fatigue builds even faster.

So this wasn’t inflammatory.

It was a system under strain.

And that completely changes how you approach treatment.

HRT made her feel worse — not better.

And this is where things often get confusing.

Because the assumption is usually the same:
👉 symptoms = low hormones

But that’s not always what’s going on.

In some women, hormone levels aren’t actually low.
The issue is how the body is processing and clearing them.

So when you add more hormones into an already overwhelmed system,
symptoms can escalate instead of settle.

This doesn’t mean HRT is wrong.
But it does mean context matters.

Supporting digestion, stress, and energy systems first
can completely change how HRT is tolerated.

I see this pattern regularly in clinic.

Did HRT make you feel worse?

This is something I see in clinic — and it can be confusing.

HRT can be incredibly effective and life-changing for many women.
But it doesn’t work in isolation.

In some cases, hormone levels aren’t actually low.�The issue is how the body is handling them.

If hormone clearance is impaired, or the stress system is already under strain,�adding more hormones can increase symptoms.

This doesn’t mean HRT isn’t appropriate.
It often means the system needs supporting first.

Once digestion, energy, and nervous system function are more stable,�HRT is usually much better tolerated — and more effective.

People are told:�“Everything looks fine”�
But they’re:�– waking at 2am�– struggling to think clearly�– bloated after meals�– running on empty

Often, the issue isn’t that nothing is wrong…�it’s that we’re not looking in the right place.

They lose weight.�They feel some improvement.
But symptoms don’t fully settle.

In this case, despite significant progress, his arthritis remained unpredictable.

When we looked deeper, there was ongoing inflammatory signalling from gut bacteria —�essentially keeping the immune system switched on.

Diet was an important first step.�It reduced load and moved things in the right direction.

But it wasn’t the full answer.

Sometimes ongoing symptoms aren’t about doing more —�they’re about identifying what’s still driving inflammation underneath.

A pattern I see frequently in clinic.

Waking at 3–4am, despite feeling exhausted.

Falling asleep isn’t usually the issue — staying asleep is.

Often, when we look a little deeper, there’s a pattern across the day:
• long gaps without food
• skipping meals
• reliance on caffeine
• larger meals later in the evening

These can influence blood sugar overnight.
When levels drop, the body responds by increasing stress hormones like cortisol and adrenaline — which can trigger early waking.

It’s rarely just a sleep problem.

Often, it reflects how the body has been supported across the day.
