This lovely lady had a basal cell carcinoma or BCC excised from her left upper lip. An island advancement flap was used to reconstruct the area and whilst things look striking in the early phases, you can see that within a matter of weeks, scars fade imperceptibly. The essential learning point is keeping a beautiful symmetry to the upper lip and not distorting the anatomy.

Another year around the table at the European Society of TMJ Surgeons (ESTMJS).

In such a niche subspecialty, it is my good fortune to be able to be part of such a club where ideas can be exchanged and we can all lean on each other to draw on a collective experience and expertise.

We get to move forwards together and keep surgical solutions for TMJ patients firmly on the map and evidence-based. We are stronger together.

As a personal aside, it is my even better fortune to call my friend. She is the soul of ESTMJS!

Botulinum toxin…..

It’s more commonly known as BOTOX, although this is a registered trademark of just one manufacturer (and my personal preference tends to be Dysport). And it’s not a wonder drug that cures all ills!

Currently the MiTiGate trial is underway to better define the place of botulinum toxin in the management of myalgia temporomandibular disorder (M-TMD). Cohort studies to date have shown mixed outcomes, with some demonstrating superiority over placebo, in one instance (Henien et al, 2024), 86% of patients cited this treatment as being “the most effective in their patient journey”, with 21% reporting complete resolution of their symptoms.

A recent trip to Amsterdam for an arthroscopy event got me talking to about its role in cervicalgia (neck pain). A recent systematic review by Bizzarri et al (2025) demonstrated higher temporomandibular disorder (TMD) prevalence in patients with neck pain (NP) and cervicogenic headache (CGH).

With this in mind, targeting neck muscles such as the sternocleidomastoid with botulinum toxin seems like a good strategy for myofascial pain and trigger points with the current scientific literature landscape. Angelo et al (2026) would seem to support this!

Just going through some reviews from the last month and this one really hit home. It is an absolute privilege to make a difference to anyone’s life to this extent. I spent a lifetime studying and training but the knowledge alone is not enough. Kindness, empathy and listening……the right diagnosis and treatment tend to follow. Also, sometimes……one person doesn’t have all the answers and collaboration is key to a successful outcome (and happy patient). Full review below!

What does a skin graft look like?

This lovely chap is a few months down the line from surgery. He had an unusual growth near the corner of his eye and the biopsy was unable to exclude cancer.

For safety, an excision was done with a margin and reconstruction performed with a skin graft taken from the neck.

Fortunately, the result was an unusually sited benign dermatofibroma, for which excision is curative. We had always discussed revising the graft with a flap, which would provide better contour and colour match from neighbouring skin, once the diagnosis was known.

But…..a few months on and he is a happy patient with a definitive diagnosis, clear margins and aesthetics he is satisfied with, the graft blending with the surrounding skin nicely and being appropriately contoured!

Proud to present the cost effectiveness study for the to the American Society of TMJ Surgeons (ASTMJS) in Ponte Vedra Beach, FL. We talked about setting the ideal price point for subscription from state healthcare providers as a cost effective alternative to standard conservative management for early stage temporomandibular disorders (TMDs) amenable to conservative (non-surgical) management strategies.

Great to network with colleagues from across the globe in a niche subspecialty and talk about potential research collaborations!

The mighty banana 🍌 blade…..

The intricacies of a level II arthroscopy of the enable a range of interventions above and beyond simple diagnostic look and lavage (washout). In this case, a prominent adhesion is tethering the disc anteriorly within the joint. Placing a second port enabled division of the adhesion but also coblation of the retrodiscal tissues.

If you have painful, clicky jaw joints with limitation of movement and pain, then let’s see how we can get things moving again with minimally invasive surgery, advanced arthroscopic techniques and limited downtime!

Such a special review…..

The pressure was on with weeks to a wedding but a time critical excision! But for me the lesson here was that surgery was secondary and for a few weeks I was probably a close second to the wedding dress designer in terms of pecking order for nuptial preparations! Supporting her through recovery was incredibly important!

But now we can all breathe a sigh of relief and this lovely lady will walk down the aisle with confidence! Very happy doctor and patient!

If you’re my patient you’ve heard the phrase “I’m better at surgery than art”……

This lovely chap had not one but two basal cell carcinomas either side of his scalp. Both were amenable to wide local excision and reconstruction with a rotation flap sometimes called “O-to-S” or “O-to-Z”.

As the name suggests (and the insets illustrate) the circular excision is closed by rotation of two flaps to the centre…..creating a “Z” (or “S”).

Not everyone wants to look like Harry Potter so meticulous closure with minimal tension means that as early as six weeks, the scars are fading fast.
