When you can probe an ulcer to bone, treat that finding seriously.

The probe-to-bone test is exactly what it sounds like. Using a sterile blunt probe at the base of a diabetic foot ulcer, you feel for a hard, gritty surface. In a foot you already suspect is infected, a positive probe-to-bone test raises the likelihood of osteomyelitis considerably.

No single test settles the diagnosis. The IWGDF and IDSA guidance treats osteomyelitis as a converging picture. A positive probe-to-bone test combined with an abnormal plain X-ray is a well established and practical combination at the bedside. Inflammatory markers support the picture, and bone biopsy remains the reference standard.



Bone involvement is common in serious diabetic foot infection, present in up to 60 percent of severe soft tissue cases. If you can touch bone, image it and escalate.

Do you probe every ulcer, or only the ones that look deep?
