After months of planning and preparation, it was wonderful to finally come together for a day filled with inspiring talks, new ideas, meaningful discussions and plenty of opportunities to connect with colleagues who share a passion for vestibular rehabilitation.

Learning through practice! 👀🧠

A glimpse from yesterday's BPPV1 course in Hereford, where participants were developing their vestibular assessment skills by practising oculomotor testing techniques on each other.

Hands-on learning is essential for building confidence and clinical reasoning when assessing dizziness, balance, and vestibular disorders.

You've achieved a level of confidence with assessing and treating posterior canal BPPV but now want to know more?

This course will teach you how to assess and treat the lateral and anterior canal variants of BPPV.

You will also learn the theory and practice behind vestibular rehabilitation for other causes of vestibular dysfunction including gaze stabilisation exercises, gaze shifting exercises, balance exercises, sensory re-weighting activities, and designing a home programme to address these skills to learn.

We will also cover common tools and instruments used to quantify dysfunction and progress including the DHI, DGI, mCTSIB, and others.

Vestibular Migraine: What Physiotherapists Should Consider Before Making the Diagnosis

🧠 Vestibular Migraine (VM) is one of the most common causes of recurrent dizziness… but also one of the most misdiagnosed.

According to the Bárány Society and NICE-aligned clinical guidance, VM should be approached carefully and only after considering other possible causes of dizziness.

Key Diagnostic Features of Vestibular Migraine

✔️ At least 5 episodes of vestibular symptoms
✔️ Episodes lasting between 5 minutes and 72 hours
✔️ Current or previous history of migraine
✔️ Migraine features during at least 50% of episodes:

headache
light sensitivity
sound sensitivity
visual aura

AND importantly:

⚠️ Symptoms should not be better explained by another vestibular or neurological condition.

Important “False Flags” Physiotherapists Should Watch For

🚩 Continuous 24/7 dizziness without clear attacks
→ Consider PPPD or functional dizziness

🚩 Positive Dix-Hallpike with classic positional nystagmus
→ Think BPPV first

🚩 Progressive hearing loss, tinnitus, or aural fullness
→ Consider Ménière’s disease or ENT pathology

🚩 Neurological signs:

diplopia
dysarthria
ataxia
limb weakness
vertical nystagmus

→ Requires urgent medical referral

🚩 Dizziness mainly on standing
→ Screen for orthostatic hypotension or autonomic dysfunction

🚩 Sudden prolonged vertigo with spontaneous nystagmus
→ Possible vestibular neuritis

Important Reminder

Patients can have:
✔️ Vestibular Migraine + BPPV
✔️ Vestibular Migraine + PPPD
✔️ Vestibular Migraine + anxiety-related dizziness

Vestibular conditions often overlap.

Clinical Takeaway

Vestibular migraine is a diagnosis based on:
🧩 symptom pattern
🧩 migraine association
🧩 exclusion of other causes

As physiotherapists, careful subjective assessment and recognition of red flags are essential before attributing dizziness solely to VM.
