What can be achieved when a care provider and local authority builds flexibility into a partnership from day one?

Since 2023, Agincare and Southwark Council have worked together to expand nursing capacity, strengthen hospital discharge pathways, support reablement services and invest in workforce wellbeing across five care and nursing homes.

From launching the Avon Unit to supporting nursing, and discharge to assess pathways, to helping residents regain independence and return home safely, the partnership continues to evolve in response to local need.

It was a lovely activity with lots of creativity and smiles as everyone prepared their special treats.

Our service users spent time updating their scrapbook and reflecting on special memories together 📖✨

They also enjoyed a friendly game of Ludo, with plenty of laughter and a little healthy competition along the way. A lovely, relaxed evening shared in great company!
