Frozen shoulder is one of the most painful conditions I see — and one of the most commonly mismanaged.
Most people are told to wait. That it will resolve on its own. And whilst that's partially true, waiting without support can mean one to three years of significant pain and limited movement.
Frozen shoulder — clinically known as adhesive capsulitis — progresses through three distinct stages: freezing, frozen and thawing. Each stage has different characteristics and requires a different approach.
Research shows that manual therapy combined with exercise produces significantly better outcomes than exercise alone or watchful waiting. A 2012 Cochrane review found that mobilisation techniques alongside corticosteroid injection produced faster improvement in pain and function than either treatment alone. More recent evidence supports a structured, stage-specific approach to rehabilitation as the most effective conservative management.
Doing the wrong thing at the wrong stage can slow recovery. But the right intervention at the right time can meaningfully reduce pain, restore movement and get you back to full use of your arm far sooner.
