One of the biggest leadership myths is that people don't know where the problems are.

Most of the time...

They do.

They know where the risks are.
They know which processes don't work.
They know where standards are slipping.
They know what frustrates customers.

And they know what leaders need to hear.

The challenge is often not awareness.

It's whether people feel able to speak.

This is where leadership becomes critical.

Because people don't usually stay silent because they don't care.

They stay silent because previous experience has taught them that speaking up changes nothing.

Or worse.

Strong cultures aren't built by encouraging people to speak.

They're built by proving that speaking matters.

There is one question that makes many leaders uncomfortable.

Not because it is difficult.

But because the answer can be.

The question is:

"What am I doing that is contributing to this problem?"

It's easy to examine systems.
It's easy to review procedures.
It's easy to identify what other people should change.

Looking inward is harder.

Yet leadership influence is often far greater than leaders realise.

The questions we ask.
The behaviours we reward.
The standards we reinforce.
The conversations we avoid.

All shape organisational outcomes.

Because that's where leadership is revealed.

When something goes wrong, organisations have a choice.

Find blame.
Or find understanding.

Look for a person.
Or look for a lesson.

Protect reputations.
Or improve systems.

The response to failure often tells you more about culture than success ever will.

People watch carefully after incidents.

They watch how leaders react.
They watch what gets discussed.
And they watch what happens next.

Because culture isn't built when things go right.

It's built when things go wrong.

I've never met a leader who woke up and decided:

"Today, we're going to lower our standards."

It doesn't happen like that.

Standards don't usually collapse.

They drift.

A shortcut here.
A compromise there.
A missed conversation.
An issue not challenged.
A concern not raised.

Over time, what was once unacceptable becomes normal.

Not because people don't care.
But because people adapt.

And that's what makes standards drift so dangerous.

It's rarely deliberate.
It's gradual.

Which means leadership isn't simply about setting standards.
It's about protecting them.

Because what gets tolerated today...
Often becomes accepted tomorrow.

Who is the most dangerous person in an organisation?

It's probably not who most people think.

It isn't the new starter.

It isn't the inexperienced worker.

And it isn't usually the person asking lots of questions.

In many cases, it's the person who has done the job for twenty years.

The person everyone trusts.
The person who knows all the shortcuts.
The person who has stopped questioning.

The person who says:
"We've always done it this way."

Experience is valuable.

But experience without challenge can become complacency.
And complacency has been a contributing factor in countless incidents across every sector.

The best professionals never stop learning.
They never stop questioning.
And they never assume that because something worked yesterday, it will be safe tomorrow.

At NHSRA, we often talk about competence.
True competence isn't knowing all the answers.

It's remaining curious enough to keep asking questions.

Because the day we stop learning...
Is often the day risk starts increasing.

Not your qualifications.
Not your experience.

You.

Would people feel valued?
Would they feel heard?
Would they feel supported?
Would they feel inspired?

Because leadership isn't something we do.
It's something other people experience.

And here's the uncomfortable truth.

Most leaders judge themselves by intention.
Everyone else judges them by impact.

Or qualifications.
Or experience.

The worst leaders demand it.

One is earned.
The other is expected.

And people know the difference immediately.

Every project is urgent.

Clarity.
They knew what mattered.
They communicated it relentlessly.
And they removed distractions.

Because people don't need more information.
They need more clarity.
