HQ Primary Care Network For patients of Hadwen Health and Quedegley Medical Centre. Since the NHS was created in 1948, the population has grown and people are living longer.

Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often. To meet these needs of our patients, Hadwen Health and Quedgeley Medical Centre are working together as a primary care network (PCN). We work with community, mental health, social care, pharmacy, hospital and voluntar

y services in the local area to proactively care for the people and communities we serve. Through the PCN, we aim to enable greater provision of proactive, personalised, coordinated and more integrated health and social care. This social media account aims to share information about and raise awareness of the many services and provisions available to our patients.
