The way we will be receiving scripts from GP surgeries is changing. We will be asking our patients to complete a consent form, if they haven't already done so. If you have any questions don't hesitate to ask a member of the pharmacy team.
To provide services under the Electronic Treatment Plan (ETP) programme in Wales, patients are required to complete the relevant forms and provide consent for their information to be shared with the community pharmacy.
The information collected allows the pharmacy team to access the details needed to safely provide your treatment, maintain accurate records, and communicate with other healthcare professionals involved in your care. Consent is an important part of this process, as it ensures that your personal and health information is handled appropriately and in line with data protection requirements.
If the completed form and consent are not provided to the pharmacy, the pharmacy may be unable to access the information required to deliver the ETP service. This could result in delays to treatment, difficulties in supplying medication, or the pharmacy being unable to provide the service until the necessary documentation and consent have been received.
Providing the completed form and consent helps ensure that your treatment can be delivered safely, efficiently, and without unnecessary delays.
