We work with vulnerable adults, such as the elderly, those who need support regarding mental health issues and others who may require additional support to remain living independently in their own homes. Supported by our excellent home care and care assistants, we are therefore working towards reduced hospital admissions through enabling people to remain in their own homes, as well as increased empowerment and social inclusion as our clients become more actively involved in the decisions which affect their future and well being, and their wider communities. Job description

We are recruiting for a Care Coordinator to join our growing team. In this role, you will effectively and efficiently co-ordinate the delivery of high quality care services across Aireborough and Wharfedale West Yorkshire , ensuring that Care Workers are matched to our service users based on the level and type of care they need, along with, when possible, being in close proximity.

Key Responsibilities:

Ensure that Care Worker personal file. On boarding of new , submitting and checking DBS and references and processed and maintained in a timely manner.

Producing fortnightly rotas

Ensure training records are updated and supervisions scheduled at the correct time.

Responsible for resourcing new care packages and building on existing packages.

Maintaining the continuity of current and new care packages.

Maintain clear, accurate, appropriate and up-to-date digital records for all service users and care workers

Responsible for cordially resolving service users and their representative’s queries in a timely manner to maintain a positive company’s reputation.

Ensure all operations are CQC and commissioners compliant.

On call duties.

Delivery and supervision of care services.

Quality assurance duties.

Medication records co-ordination.

Contribute to the growth and service expansion targets set by the business.

Requirements:

Minimum NVQ/QCF level 3 in Care/Level 5 would be advantageous

At least 2 years’ experience working in home/residential care

Knowledge of CQC Compliance is essential.

Ideally experienced in conducting care assessments, care reviews

Liaising with multidisciplinary teams.

Organised, methodical, and a good eye for detail.

Being flexible around working schedules, roles and responsibility.

Being a team player is extremely crucial to this role.

Job Summary
We are seeking a dedicated and organised Senior Care Worker. The successful candidate will be responsible for carrying out initial assessments coordinating digital person cantered care plans, raising the company profile within the community and multi-disciplinary teams. assessment supervising care staff, raising and ensuring the well-being of service users. This role offers an excellent opportunity for individuals with leadership experience and a passion and wanting to progress to a more senior role within the organisation and to make a meaningful difference in our service users' lives.

Duties
Develop, implement, and review personalised care plans tailored to each of our service users’ individual needs.
Supervise and support care staff to ensure the delivery of compassionate and professional care.
Liaise with families, healthcare professionals, and external agencies to ensure seamless communication regarding resident care.
Monitor service user’s’ progress and outcomes and adjust care plans, accordingly, maintaining accurate records using IT systems.
Organise training sessions and supervise staff development initiatives to uphold high standards of care.
Ensure compliance with health and safety regulations within the care environment.
Cover care calls in the community and covering sickness and holiday cover.

Experience
Proven supervisory experience within a homecare or senior care environment.
Strong leadership skills with the ability to motivate and guide multidisciplinary teams.
Experience in developing and managing care plans tailored to individual needs.
Excellent communication skills, both verbal and written, with the ability to liaise effectively with residents, families, and professionals.
Competent IT skills for managing documentation, scheduling, and reporting tasks.

Valid driving licence is desirable for travelling between sites or external meetings.

This role is ideal for proactive individuals who are organised, compassionate, and committed to delivering exceptional senior care through effective coordination and leadership.
