The importance of looking at things globally

When I assess patients I don’t just look at where they are feeling pain, I look at the other joints / tissues and also look for movement errors which may cause problems further up the chain.

For example when I assess a shoulder, I will also look at the neck, upper back and scapula for things which may be contributing to the issue itself, not all injuries occur in isolation, they are often multifactorial.

In this particular case, a chronic lower back problem in the sacroiliac joint (SIJ) and lumbar spine, which mainly affected the left side, was found to be caused by a movement error at the hip for to weakness in the gluteus medius and TFL, so when the patient stood on one leg, greater stress was placed on the SIJ and lower back. No amount of manipulations, massage or mobility work will resolve the issue fully as long as the underlying issue wasn’t diagnosed and addressed directly!

Psychological implications of injury - Kinesiophobia

Kinesiophobia is fear of movement often assisted with injury where people will avoid certain movements due to fear of injury.

It is an obstacle I often need to overcome with patients throughout the rehab process, building confidence with movement through education, progressive overload, and most importantly… listening! Hearing what is it the patient has concerns about and working together to overcome these fears.

Pain doesn’t always mean damage, it is sometimes a normal part of the rehab process within limits, and not all movement is dangerous. Rehab is training in the presence of injury, this is where I work closely with patients to get them training safely whilst injured, and building their tolerance to the demands of the activities they will eventually be returning to.

We rehab people, not just the injured tissue, so by the end of the rehab process they are returning to sport / activity with full confidence where performance isn’t hindered.

ACL rehab week 7 ⚡️

Proof that hard work pays off! 💪 Through applying
progressive overload into strength training and hitting targets, we are now in a position to start implementing single leg loading! 🦵

This isn't determined by time scales or guesswork, these decisions are made through objective & subjective testing! Without data, you're just guessing! 🤷‍♂️

Alongside this, we are other goals we are working to achieve such as balance, movement quality, restoring mobility equal to the uninjured knee. 📈

There is also careful rehabilitation of the hamstring following a hamstring graft. After a few weeks of relative protection, then a lot of isometric work, and now loading the hamstrings through movement following progressive overload principles.

Surgery is the easy part, the hard work starts immediately afterwards, and requires a lot of mental resilience and commitment! 🏋️

Yesterday marks 1 year since graduating with a BSc (Hons) In sports rehabilitation from the university of Salford 🎓

To say it’s been an intense year would be an understatement! However, throwing myself in at the deep end was the best thing I could have done to gain experience, implement my knowledge, and develop my own practice. Having worked with Tyldesley Rugby Club and since July last year, and then opening my own injury clinic at The Yard Gym in October, I’ve had the opportunities to really help people get back to what they love doing most. I’ve managed many injuries such as:

Multiple ligament injuries including ACL, MCL, ATFL, AC joints and so on…

Multiple fractures such as fibula, clavicle, metacarpals etc…

Dislocated fingers, shoulders and ankles

More than enough wounds!

And interesting conditions such as Osgood Schlatter, Chondomalacia patellae, Severs disease and more.

Dealing with these in acute settings and long rehabilitation journeys, there is no more rewarding feeling than being with people through some of their worst times, as well as the best when they get back to where they want to be.

Forever grateful for the opportunities I’ve had so far in my career, and looking forward to where the future leads. Meeting and working with some fantastic people.

We’re delighted to have Alfie as part of the Tyldesley RUFC team.

From day one, Alfie has created a professional and welcoming environment, becoming a valued member of our coaching and support staff. His knowledge, dedication and genuine care for the players have played a huge role in keeping the squad fit, healthy and performing at their best throughout the season.

“I’ve loved my time working at Tyldesley RUFC. There is a great environment at the club where everybody makes you feel welcome!
