One Year Later… What a Difference! 👣👌

This young patient attended Lincoln Footcare with a painful ingrowing toenail affecting his right big toe.

A sharp spike of nail was digging deeply into the surrounding skin, causing pain, inflammation and considerable discomfort.

When an ingrowing toenail repeatedly penetrates the skin, simply trimming the visible corner may provide only temporary relief. A hidden nail spike can remain within the side of the toe and continue to cause irritation.

Following a full assessment and discussion of the available treatment options, a partial nail avulsion—or PNA—was performed.

This precise procedure removes only the troublesome section of nail while preserving the healthy remainder of the nail plate.

The treatment included:

✅ Local anaesthetic to keep the toe comfortable
✅ Careful removal of the painful nail spike
✅ Preservation of the healthy section of nail
✅ Professional dressing and aftercare
✅ Follow-up support throughout healing

And the second photograph shows the result one year later.

The toe has healed beautifully, the painful nail edge is no longer causing trouble, and the remaining nail looks healthy and natural.

This is a lovely example of how partial nail surgery can provide lasting relief while still achieving an excellent cosmetic result.

Pressure Is Only Half the Story… 👣

When we analyse your gait, we don’t just look at how much pressure your feet experience — we also look at how long that pressure is applied.

This is known as plantar pressure impulse, and it can give us valuable information about how your feet are loading during every step.

You may not have the highest peak pressure in a particular area, but if that area remains loaded for longer, the repeated cumulative stress can contribute to irritation and overload of the joints, tendons and soft tissues.

- Retronychia – When the Nail Grows Backwards 👣

Not all “ingrown toenails” grow down the sides of the nail.

Sometimes the nail can become traumatised or disrupted and start growing backwards into the skin at the base of the nail, near the cuticle. This condition is known as retronychia.

Retronychia involves the nail plate being pushed into the proximal nail fold — the skin at the base of the nail. This can lead to ongoing irritation, inflammation and pain.

Common signs can include:

• Pain at the base of the nail
• Redness and swelling around the cuticle
• Bleeding or discharge
• Thickened or distorted nail growth
• Layers of nail appearing to build up
• Difficulty wearing shoes comfortably
• Recurrent inflammation that doesn’t fully settle

It is often mistaken for a standard ingrown toenail or infection, but the problem is usually coming from the base of the nail, rather than the sides.

In some cases, antibiotics alone may not resolve the issue because the nail itself continues to irritate the surrounding tissue.

At Lincoln Footcare, we assess the nail carefully to identify whether this is a typical ingrown toenail, paronychia, trauma-related nail damage, or possible retronychia.

Treatment depends on the severity, but may involve removing the offending nail plate to allow the area to heal properly and prevent ongoing irritation.

✨ Sculptra® Treatment – Fat Pad Atrophy (1st MPJ) 💉

This patient presented with fat pad atrophy beneath the 1st MPJ (big toe joint) — experiencing pain and tenderness on the ball of the foot, particularly during walking and prolonged weight‑bearing.

Over time, the natural cushioning beneath the forefoot can thin, leading to increased pressure over bony prominences such as the 1st metatarsophalangeal joint.
This can result in:
• Localised pain
• Painful Corn’s
• Difficulty in footwear
• Callus formation
• Reduced shock absorption

To help restore cushioning and improve comfort, we carried out Sculptra® dermal filler treatment.

Sculptra works by stimulating collagen production, gradually improving tissue thickness and providing additional padding in areas affected by fat pad loss.

For patient comfort, we performed a tibial nerve block, ensuring the foot was fully numb before treatment — meaning no pain during the injection itself.
