💙 The SWAGGA Way – Better Care, Closer to Home! 💙

At SWAGGA PCN, we’re always looking for new ways to improve services, make healthcare more accessible and give our patients and communities the best possible care. And we’re proud to be making a difference! 👏

Yesterday, we had the pleasure of welcoming Professor James Kingsland to discuss the future of neighbourhood health across the NHS and showcase how The SWAGGA Way is already making a real difference to the people we serve.

And yes… we think he left feeling a little SWAGGA-rised! 💙😊

So, what does this mean for you as a patient?

We know that patients don’t live their lives around NHS boundaries or organisational structures. You want care that is accessible, joined-up and centred around you – and that’s exactly what we’re working towards.

Our journey as a Primary Care Network has already come a long way, but we’re not standing still. We’re continuing to build on what works, challenge what could be better and explore new opportunities to improve the care and support available across our communities.

Because for us, neighbourhood health isn’t just about changing how the NHS works. It’s about making a real difference to your health, your wellbeing and your experience of care. 💙

We wanted to highlight the incredible work of Love Jasmine, a charity supporting families following the loss of a child — from pregnancy loss through to the loss of an adult child.

They provide a wide range of support including counselling, support groups, workshops, relaxation sessions and much more. Their work has made a huge difference to so many families across our communities.

Sadly, Love Jasmine has recently lost a big amount of funding and are currently working hard to raise vital funds to help keep these important services available.

There are lots of ways you can help from supporting their fundraising activities to simply following, sharing and spreading the word about the work they do.

We're particularly proud to be supporting Marie Curie Liverpool in raising awareness of the incredible services they provide for people with a progressive or life-limiting illness and those closest to them. From social activities and wellbeing groups to carers' support, complementary therapies, exercise sessions and specialist advice, Marie Curie offers so much more than many people realise.

At SWAGGA PCN, we never stand still.

We are committed to continuous progress, evolving, listening, and building partnerships that ensure our Neighbourhood Health Hub delivers what matters most to our patients.

Our directors, Dr Coleen McCabe and Gina Barnett, are relentless in their efforts to collaborate across the system. In a recent meeting, they were joined by Professor Matt Ashton (Director of Public Health), Chris McBrien (Associate Director of Public Health), and Cllr Richard Kemp to discuss the future direction of neighbourhood health.

The conversation was incredibly positive, with strong public health and councillor support for SWAGGA’s vision. There was a shared commitment to help escalate key issues at both council and ICB level, ensuring the voices of our communities are heard.

A few key themes stood out:
Neighbourhood health needs sustainable funding – without an adequate financial framework, long-term impact is limited.

Clarity is essential – the term “neighbourhoods” is being interpreted differently across sectors, and alignment is critical for effective delivery.

Partnership is everything – meaningful change only happens when organisations work together with a shared purpose.

But perhaps most importantly…
Primary Care cannot afford to be passive.

At SWAGGA, we believe Primary Care must be at the forefront of designing and developing the system, not reacting to it. PCNs across the country should not wait to be told what the model looks like, we should be showing the system what is possible.

We aim to demonstrate:
The impact neighbourhood models can have for residents and patients — the people who matter most

The cost efficiencies and savings that can be achieved through integrated, proactive care

The real, scalable solutions that come from working together locally

This is about leadership, innovation, and putting communities first.
