We’re feeling a mix of excitement and nerves as we get ready to open the doors to SWAGGA’s Neighbourhood Health Hub tomorrow at 9:00am 🎉

This moment has been a long time in the making. Not just the past few weeks of intense preparation, but the vision we’ve been building over the last few years is finally becoming a reality. So much hard work has gone in to get us here — from planning and partnerships to the final touches this week… even our Managing Director, Gina, has been scrubbing the toilets to make sure everything is perfect! 💪🧼

We want to say a huge thank you to everyone who has reached out with well wishes and offers of support — it truly means a lot and reminds us why we’re doing this ❤️

A special thank you also to South Liverpool Homes for their generous donation towards refreshments on the day — we really appreciate your support 🙌

🎉 GRAND OPENING – SWAGGA PCN Neighbourhood Health Hub at South Liverpool Treatment Centre!🎉

We are delighted to invite EVERYONE to the official grand opening of the SWAGGA PCN Neighbourhood Health Hub based at South Liverpool Treatment Centre on:

📅 Monday 1st June
⏰ 9:00am

This exciting new space has been created to support the health, wellbeing and connection of our local neighbourhoods – and it’s just the beginning!

SWAGGA PCN is passionate about ensuring that the Hub truly reflects the needs and voices of the people it serves. That’s why we’ve been working closely with local leaders, including MP Maria Eagle, Cllr Sam Gorst, and Liverpool Health & Wellbeing Board member and Cllr for Penny Lane Ward, Cllr Richard Kemp CBE, to help shape its vision.

But most importantly… this is your Hub. 💙

We want to hear directly from you—our patients and residents. In the coming weeks, we’ll be hosting consultation days with our directors, giving you the opportunity to share your ideas, experiences, and priorities for what services and support you want to see most.

👉 If you can’t make a consultation day, don’t worry—just pop into the Hub and speak with a member of the team, who will ensure your ideas are shared. You’ll likely even catch one of our dedicated directors in the Hub and can speak with them directly.

🌟 Exciting things are happening behind the scenes at SWAGGA PCN! 🌟

We’ve been working on something truly transformative for our neighbourhoods… and we’re finally getting closer to sharing it.

Last week, we were delighted to welcome Cllr Sam Gorst to hear all about the exciting developments soon coming to the SWAGGA PCN Network – and effectively, to Garston. One of the biggest announcements?

🏥 SWAGGA PCN will be opening a Neighbourhood Health Hub.

A space designed to bring services together, strengthen local support, and make it easier for patients to access the care and community resources they need.

It was fantastic to hear just how enthusiastic Cllr Gorst was about the project. His commitment to helping bridge the gaps between Primary Care and local services will play a vital role in shaping a healthier, more connected community for everyone.

Today we’re celebrating the amazing work of Social Prescribers — the professionals who support people with their social, emotional, and practical wellbeing.

👥 Did you know?
Social Prescribers have been working within communities for many years, but many people still don’t know who they are or how they can help. They’re here to connect you with non‑medical support that can make a real difference to your health and happiness, such as:

Local groups, activities, and community resources
Support with housing, benefits, or financial worries
Help with loneliness, stress, or emotional wellbeing
Lifestyle advice and wellbeing programmes

🤝 Working Together for Our Communities
SWAGGA PCN have been working collaboratively with Health Junction to provide high‑quality Social Prescribing support for our residents. Together, we’re making sure everyone has access to the guidance and services that help them thrive.

✨ Accessing Support Has Never Been Easier
You can now self‑refer directly to our Social Prescribing Team.
That means:
✔️ No need to wait on the phone
✔️ No need to book a GP appointment

Simply fill out the referral form via the link or QR code, and one of our Social Prescribers will get in touch to book you in — face‑to‑face or over the phone, whichever suits you best.

🌟 SWAGGA x Pumping Marvellous BEAT Event — Speke 🌟

What an incredible day supporting the BEAT cardiovascular screening event at Speke Neighbourhood Centre! The community impact was HUGE 👇
❤️ 380 people screened
• 10 high blood pressures
• 6 new AF cases

🩺 39 NHS Health Checks
• 14 raised cholesterol
• 10 raised glucose

🔍 24 patients had proBNP tests → 14 raised markers
AI echo confirmed:
• 6 new heart failure cases (1 urgent same‑day referral)
• 6 new AF
• 1 Amyloidosis
• 1 valve defect

✨ The most powerful part?
Patients were able to be screened, diagnosed and started on medication — all on the SAME DAY.

No long waits.
No delays for tests.

Just instant answers and peace of mind when it mattered most.

A huge shout‑out to all partners providing full wrap‑around support: NDPP, Smoke Free Liverpool, Lung Cancer Screening, Talking Therapies, River, Diabetic Eye Screening, Live Life Liverpool, Access Wellbeing and SWAGGA’s own Ageing Well & Cancer Screening teams 💛

🌟 SWAGGA Proudly Supporting the Pumping Marvellous BEAT Event in Speke! 🌟
SWAGGA was proud to support the latest Pumping Marvellous BEAT cardiovascular screening event at Speke Neighbourhood Centre — a truly vital and much‑needed day for the local community.

The impact was incredible!!
❤️ 380 people screened through blood pressure checks, heart rhythm monitoring and a breathlessness questionnaire. This identified:

• 10 significantly high blood pressures
• 6 newly detected cases of Atrial Fibrillation (AF)

🩺 39 full NHS Health Checks were also completed on the day, uncovering:
• 14 people with significantly raised cholesterol
• 10 people with significantly raised glucose levels

🔍 From the initial screening cohort, 24 people with raised BP or newly detected AF received a proBNP test to check for markers of potential heart failure.
Of those 24:
• 14 had elevated markers
• Each received an AI echocardiogram on the day, which confirmed:

6 new heart failure diagnoses (including 1 urgent same‑day referral)
6 additional AF diagnoses
1 case of Amyloidosis
1 patient with a valve defect

✨ What made this event truly remarkable was the ability for patients to be screened, receive a diagnosis, and even start medication — all on the same day.

No waiting months for tests.
No delays for results.

Just same‑day clarity, reassurance, and peace of mind at a time when early intervention matters most.

These findings are life-changing — and in many cases, life‑saving. The event showcased the true value of proactive, community-based screening, identifying serious health conditions before symptoms escalate and helping people access treatment when it can make the biggest difference.

Join us for a FREE Heart Health Check and take a positive step toward protecting your long‑term health.

This event supports the B.E.A.T. Heart Failure Programme, helping people understand the signs of heart failure early using four simple steps:

B – Breathlessness
E – Exhaustion
A – Ankle swelling
T – Time to get your heart checked

Our goal is to help more people recognise symptoms sooner and stay healthier for longer. Your check could make a real difference.
