Playlist for Procedures by Shirish Prabhudesai

Before most procedures, I try to ask patients what they would like to have playing in the background.

My experience in training so far is that, for many procedures performed under local anaesthetic or conscious sedation, the right music can make a real difference for patients who are understandably anxious about the experience.

When patients do not have a particular preference, I often return to a playlist of calming pieces from different periods, ranging from the likes of Sibelius to Paul Mealor.

One contemporary piece I often start with is an instrumental arrangement of “Always with Me” / “Itsumo Nando Demo”, the closing song from Spirited Away, performed by the Czech Philharmonic Chamber Orchestra.

I like opening with this piece because it immediately creates a calm and reassuring atmosphere without demanding too much attention.

The arrangement carries the gentle lyricism of the original song into a broader symphonic landscape. Delicate, nostalgic and quietly moving, it has an almost lullaby-like quality, capturing the emotional afterglow of the film: the sense of returning home changed, with memories of a strange and beautiful journey still lingering.

In the setting of an interventional radiology procedure, that tone feels particularly fitting. Music in the room should never dominate the clinical environment, but it can help soften it. This piece has a steadiness and warmth that can ease the transition into the procedure, giving both patient and team a quieter emotional starting point.

For me, it works well as an opening track because it is gentle, familiar to some, unobtrusive to others, and emotionally grounded without being distracting. It helps set the mood I want at the beginning of a case: calm, focused and humane.

The British Society of Interventional Radiology (BSIR) exists to advance the practice of Interventional Radiology (IR), improving patient outcomes, enabling IRs to enhance their practice, and providing a stronger voice for IR within the healthcare system.

Our long-term strategy focuses on five key goals:

1️⃣ Improving patient care and safety: We're working towards ensuring all Trusts/Health Boards provide allocated time for IRs to see patients properly in outpatient clinics.

2️⃣ Increasing recruitment to IR: We aim to create a clear IR identity separate from Diagnostic Radiology (DR) to attract more trainees and professionals.

3️⃣ Enhancing IR education and training: With a focus on creating dual certification, we strive to produce a curriculum designed by IR specialists.

4️⃣ Building IR research capability: Stimulating IR-led research is key to providing evidence-based practices and moving IR procedures into treatment guidelines.

The RAVE trial is a major NIHR-funded randomised trial looking at an important question in endovascular lower limb revascularisation:

Is intravascular lithotripsy superior to plain angioplasty or other vessel preparation methods?

In this short video, Thanos Saratzis, CI of the RAVE trial, explains who may be suitable for the study and how sites can get involved.

One of the key messages is that RAVE is designed to be pragmatic and effectiveness-driven.

That means patients may be considered if they are undergoing endovascular lower limb revascularisation and the clinician or MDT feels IVL could be used as part of their treatment.

The trial is also NIHR portfolio supported, with recruitment due to commence in late 2026.

Costs for devices used within the trial are covered through the NHS/NIHR, and follow-up is aligned with usual NHS practice.
