On 12 August, the UK gets a rare partial solar eclipse 🌒 — the biggest one we'll see since 1999.

But here's what I want you to know before you look up: your sunglasses won't cut it. The eclipse tricks your eyes into feeling comfortable looking at the sun, which is exactly what makes it dangerous — retinal damage from UV exposure happens silently and painlessly, so you often don't notice until it's too late.

✅ Genuine eclipse glasses (look for ISO 12312-2 certification, no scratches)
✅ Indirect viewing — pinhole projector, colander, even shadows through tree leaves

❌ Regular sunglasses, no matter how dark
❌ Damaged or uncertified eclipse glasses

⭐️Before Vs After⭐️

Here is a lovely example of how squint surgery can change someone's happiness and self-esteem. This very sweet 10 year old boy underwent unilateral squint surgery on the left eye. The photo at the bottom is his eye alignment 2 weeks after surgery. He was so pleased and grateful we were able to help him regain confidence. Needless to say, this makes my job worthwhile.

⭐️Epidermal inclusion cyst ⭐️

Here is a 10 year old child with a slow growing cyst, which was freely mobile with no overlying skin changes. I removed this cyst intact, and it was sent for biopsy. The result was an epidermal inclusion cyst. Pleased to say the parents and child were very happy with the result.

Here is a 16 year old girl with a 2 month history of reduced vision. She was known to have posterior subcapsular cataracts (most likely congenital). She had previously good visual acuity, but on presentation to her optician, the vision in her left eye had dropped from 6/7.5 with pinhole to HM. As you can see, she has a complete white cataract in the left eye, which has progressed rapidly. There was no history of trauma. On examination, her left cataract was intumescent and white, blocking the fundal view. Ultrasound B Scan was performed, which showed a flat retina and no vitreous changes. Swipe ➡️ to see the anterior segment OCT, which shows the protrusion forwards of the lens. Thankfully, intraocular pressure was normal.

The child that can not see.

This 7 year old boy presented with reduced distance and reading vision for several months. See the fundus autofluorescence above showing a dark choroid surrounded by hyperfluorescent pisciform flecks. Swipe ➡️ to see the fundus photos showing the characteristic Bull's eye Maculopathy. This child was diagnosed with Stargardts disease. This is an autosomal recessive disorder of the ABCA4 gene, causing photoreceptor dysfunction, resulting in the accumulation of lipofuschin. Unfortunately, this condition has no cure, but treatment is aimed at supportive visual aid.

⭐️Juvenile Idiopathic Arthritis (JIA)⭐️
Here are the typical 'mutton-fat' keratic precipitates or KP's seen on the corneal endothelium in granulomatous uveitis. This 11 year old girl reported bilateral blurred vision with no pain or redness. This is the typical presentation of uveitis in children with JIA, hence the need for regular screening. This girl was immediately started on intensive topical steroids and cycloplegic drops to prevent progression and complications.

⭐️'My Daughter is a different person'.⭐️

These are the words of the mother of the girl in the photo above, 2 weeks after her squint surgery. I urge those who feel squint surgery is a 'cosmetic' procedure to talk to this mum. She told me (with tears in her eyes) that she hadn't realised the psychosocial impact the squint was having on her child. From teasing by other children to teachers telling her off for not concentrating in class, these repeated experiences have the ability to shape the outcome of each child. It can affect self-esteem, social interactions, and a child's ability to thrive in society. I am so glad we helped this child overcome her social anxieties, and hopefully, this will impact her life in a positive way going forward.

⭐️Teaching & Training⭐️

Today we set up strabismus simulation training for some of our doctors. This involved practising surgical techniques for recession and resection of muscles on a model eye. I always find it best to start squint surgery on these model eyes to get a feel of the procedure and to develop familiarity with the surgical instruments. We had great feedback from our doctors and they found this approach much more valuable as we were able to take our time and teach them tips to make the procedure easier.

Rising up to the challenge!

Today I had to literally and figuratively rise up to the challenge in front of me. I performed a cataract operation standing up! For those that may not know, cataract surgery is usually done by the surgeon sitting in front of the microscope and using both legs to control the foot pedals of the microscope (right foot) and ultrasound machine (left foot). However, this patient had a chronic lung condition and back surgery which meant she was unable to lie flat, therefore, we had to adjust our technique in order to acheive the best outcome for the patient. The surgery was successful and without complications so a big win all round.
