I am developing, trialing and refining, a free self assessment and treatment algorithm for low back pain patients. It takes patients through self assessment, so they are referred to A&E, or a consultant/ GP where appropriate. If this is not essential it takes them through questions and self assessment to give them a starting point and an exercise and education programme. In time the aim is for thi

s to be an automated website, but for now as it changes almost daily I'm emailing out questions and refining day by day! If you would like to try the algorithm please contact me through this page. Kieran is a physiotherapist, nutritional therapist and corrective exercise specialist who specialises in comprehensive evidence based rehabilitation of spinal pain patients. Kieran has worked in the health, exercise and rehabilitation industry since 2004. Starting as a personal trainer and sports massage therapist consulting private clients in a gym environment, he has since gone on to complete a physiotherapy degree, diploma in nutritional medicine and advanced studies in holistic lifestyle management and corrective and high-performance exercise. This has enabled him to work with a wide range of patients suffering with acute and chronic conditions. Kieran presents regularly to health care professionals on a diverse range of topics and undertakes scientific reviews for educational institutions, private clinics and individuals. These have varied from the role of inflammation to the development of trigger points and the manipulation of acute exercise variables on health, function and performance. In addition he has a clinical interest in chronic pain management, Parkinson’s disease and the management of irritable bowel syndrome.

Evening blue light from phones, tablets, and TVs suppresses melatonin, the sleep hormone. It might not stop you falling asleep, but it can shallow your sleep and increase night-time waking (Cajochen et al., 2011; Chang et al., 2015).

In some people who develop chronic pain the immune response after injury never settles back down to normal. One cause of this is omega 3 deficiency, as omega 3’s are required to produce "resolvins", the molecules that resolve the inflammatory response (Lorente-Cebrián et al 2015). Are you eating mackerel, salmon, sardines, flax seeds or chia seeds most days?

If I was to recommend one nutritional test to get done each year it would be a nutrieval by Genova. This looks at everything from your vitamins and minerals, omega 3's and other fatty acids, energy production, heavy metals, plastic exposure and basic markers of gut health.

Among homeopathic remedies, arnica has the best evidence. Topical gels or creams may help with bruising, swelling, or acute soft tissue injuries, though benefits for chronic low back pain are small and inconsistent (Kouzi & Nuzum 2007; Oltean et al., 2014).

If I were to recommend one form of yoga it would be Iyengar. Iyengar focuses on doing static positions well and to me is just good physiotherapy. BKS Iyengar himself was capable of doing handstands and other very advanced exercises deep in to his 90s.

Good nutrition, fruit veg and protein (vitamins A, C, D, zinc, selenium, protein) helps vaccines work better, while heavy alcohol use weakens immune response. Moderate alcohol doesn’t seem to impair vaccine effectiveness—but avoid binge drinking before and after your shot (Gombart et al., 2020; Mossad, 2021). If you're getting a vaccine eat like an adult before and after.

Magnesium plays an important role in calming us down. It is essential for the GABA receptors to work, GABA is a "calming" neurotransmitter (Abbasi et al., 2012, Boyle et al., 2017). As such when we get deficient some people feel more anxious and may struggle to sleep. A magnesium based sleep formula such as nutri advanced calmeze can be very effective at helping people in this situation.

IL-17 is a powerful driver of inflammation. Elevated IL-17 is associated with joint and back pain, morning stiffness, enthesopathy, skin inflammation (e.g. psoriasis), gut symptoms, fatigue, and heightened pain sensitivity (McInnes & Schett, 2011; Blauvelt & Chiricozzi, 2018). Diets high in salt, sugar, and saturated fat increase IL-17 via Th17 immune activation, while fibre, omega-3 fatty acids, vitamin D, and polyphenols reduce IL-17 by improving gut microbiota and immune regulation (Kleinewietfeld et al., 2013; Calder, 2017; Jeffery et al., 2016). Eat like and adult.

Injuries can prevent us from doing things that provide us with positive endorphins, social interactions, health benefits and many other positive results. If an injury is stopping you from doing something, try to establish what benefits you are not getting, calorie burn, social interaction etc and put activities in place to provide the lost benefits.

Lack of self control is associated with worse outcomes from COVID and many other conditions and diseases (Bogg & Roberts, 2004, Daly et al., 2021). The Stoic philosopher Musonius Rufus 30AD-100AD said mastering control of your diet was the first step to self control, and this might be a good place for a lot of us to start
