Meeting pour discuter du cas d’une patiente entre le Dr Fern (orthodontiste) et le Dr Sadone (Dentiste) pour trouver une solution

Laura: la patiente a 17 ans; elle n’est pas motivée pour des bagues et elle a la deuxième prémolaire en haut à droite qui est à l’intérieur du palais; elle souhaiterait une solution alternative.
On a également enlever les dents de sagesses

Fern : Est-ce qu’elle a déjà eu des bagues avant?

Laura: non, aucun traitement orthodontique n’a encore été fait. Voici les photos

Fern: Oui la prémolaire est très rentrée et il y a aussi un inversé d’articulée avec la dent équivalente sur l’arcade du bas quand elle ferme la bouche.

Si la continuité de l’arcade n’est pas maintenue et qu’une dent part vers l’intérieur, c’est toute l’arcade qui en subit les conséquences. Regardes, les dents d’à côté ont commencé à se déplacer et des espaces se forment. Progressivement, le milieu incisif va se décaler et toutes les autres dents avec, avec le temps.

L’idéal c’est de l’orthodontie, sinon peux-tu sur cette dent faire une couronne et recaler la position de la dent en résolvant l’inversé d’articulée ?

Laura: Non ce n’est possible ; La dent est bien trop rentrée à l'intérieur pour rattraper la position avec une simple couronne. La seule solution pour rétablir l’alignement serait d’enlever la dent et de prévoir un implant. Je voudrais voir avec toi comment on peut éviter ça.

Fern: Bien sûr il vaut mieux de l’orthodontie mais si la patiente n’en veut pas et qu’elle préfère un implant, il faut aussi lui conseiller une attelle collée de contention derrière les dents de devant pour éviter que les dents continuent de bouger et perdent leur alignement.

Laura: la patiente ne veut pas de bagues mais si l’orthodontie est la meilleure solution, est-il possible de faire le traitement avec des gouttières transparentes Invisalign ?

Fern: bien sur, Invisalign a beaucoup évolué et maintenant c’est possible de résoudre ce cas avec Invisalign. Par contre avec Invisalign il sera possible de ramener la dent sur l’arcade mais il sera plus difficile qu’avec des bagues de corriger la rotation de la dent. Il faudra alors voir après le traitement d’orthodontie s’il ne sera pas nécessaire de faire une couronne sur cette dent pour s’assurer que l’engrènement dentaire entre l’arcade du haut et du bas soit correct.

An orthodontist should see a child at an early stage when all baby teeth are in place.
