We are pleased to present our 2025/26 Annual Report, highlighting a year of achievements and ongoing commitment to reducing inequalities and improving health and social care services in the borough.

The report showcases the impact of our projects and how health and care services have improved from listening to your experiences. Your feedback helped lead to practical improvements in GP practices and informed strategic plans on neighbourhood working, hospital discharge, mental health care, digital services, and more.

The report also shows how we have worked with specific communities to create change. Findings from our projects with Black men with a Serious Mental Illness and people experiencing homelessness and dual diagnosis have both led to strategic commitments for improvement.

What should neighbourhood care look like in Lambeth?

Over 350 residents took part in workshops, interviews, and a survey to tell us what they would like to see from Integrated Neighbourhood Teams (INTs).
People told us they strongly support the idea of Neighbourhood Teams – people want community-based care that is less fragmented and more accessible, timely, and holistic.

They also emphasised the importance of trust and how digital tools should complement, not replace, in-person support.

Most importantly, neighbourhood care should reflect the different priorities and needs of Lambeth’s diverse communities.
