Our partners at SE London ICS are building a brand-new digital version of the Vital 5 Check—and they want your help to design it so it works perfectly for everyone in our community.

What is the Vital 5?
It's a quick, free check-in that helps you understand 5 key areas crucial for long-term health:
💓 Blood Pressure
🚭 Stopping Smoking
🍺 Safe Drinking
🧠 Healthy Mind
🍏 Healthy Weight

By sharing your views in a quick survey, you can directly shape how this new digital service looks, feels, and functions for patients across Greenwich and South East London.
