Industry bodies and individuals - including Professor Sir Bruce Keogh and the JCCP - have hammered home the urgent need for aesthetic regulation in England as we mark 13 years since the Keogh Report, with barely a shred of progress.⁠

An open letter was sent to the Secretary of State for Health & Social Care, Yvette Cooper, MP, on 6th August 2026. It highlights not only the ongoing lack of regulation, and no aesthetics licensing scheme, but the incredible risk patients are being put at by this and some surgical loopholes - whereby any GMC-registered doctor can legally perform cosmetic surgery - which need immediate attention.⁠

When what you say on social media is more heavily policed than the treatments themselves, there is a clear problem. ⁠

💡 You may have seen this talking point online recently, around masseter botox causing jowls and sagging in the lower face. But is it true? Not really, although it's based in fact.⁠
⁠
⭐️ This is a great example of why patient selection, understanding contraindications and possible risk factors, plus tailored treatment planning approaches are key for all aesthetic practitioners.⁠
⁠
❌️ If your patient has youthful, elastic skin, masseter toxin is unlikely to cause jowling⁠
⁠
📉 But where your patient is older, has skin laxity, collagen loss and/or pre-existing jowling, masseter botox may exacerbate this lower face sagging. This is because you're shrinking the muscle that was essentially acting as scaffolding, allowing for this downward shift⁠
⁠
💉 So what do you do if this happens to your patient? A multimodal approach is usually best, addressing jowling with filler or a stimulatory treatment such as Profhilo Structura⁠
