What makes a great physio? Here are a few signs to look for

If you leave the session feeling heard, clear on the plan and confident in what comes next, you’re likely in the right place

Heel raises alone do not reflect the true demands of running or sport

Using them for clearance, even when weighted, can lead to premature return and setbacks

Testing should meet and exceed the loads you’re returning to. This often involves a combination of plyometric, load tolerance and strength capacity

A meniscus tear is not just a meniscus tear

🔎 Some are irrelevant. Some are silent. Some change everything

There are many different types and that distinction matters

Surgery has a role and is the absolute right option for some but many do very well without

With so many different variables it’s impossible to cover everything in one post but here’s a few key takeaways

✔️ Clinical decisions should be individualised
✔️ Type + location > ‘just a tear’
✔️ Rehab should be first-line in most cases
✔️ Don’t miss complete radial or root tears
✔️ True locking matters (think displaced/bucket-handle)
✔️ More tears should be repaired not removed
✔️ A ‘quick clean up’ is likely not the answer
✔️ We may never know all the answers

Are you sure you need a scan?

🩻 Scans (x-ray, MRI etc) are often seen as the ‘answer’ to pain but the truth is they usually aren’t

A scan shows structure, not pain. These changes on scans don’t always match how you feel

This isn’t an anti-scan post as they can be very helpful when used for the right reasons and explained properly

Problems arise when results are given without context - when you’re shown results in black and white, or worse, left to figure them out on your own

🫡 When good physios say no to a scan, they’re not blocking you, being stubborn or cutting costs, they’re focusing on what really matters: your recovery
