Behind the scenes of building an ACL return to sport for a female football player.

ACL return to sport is a programming problem, not just a rehab problem.

Strength. Power. Plyometrics. Running exposure. Deceleration. Change of direction. Conditioning.

There’s too much to train well and recover from in a single 7-day cycle.

So for this footballer, it’s better building an A/B week structure across 14 days. Different priorities, different exposures, enough recovery while still training hard.

The goal isn’t just a strong knee.

We have three huge announcements at the clinic which we will share with you all soon.

Number one is this huge upgrade to our clinical staff!

From Tuesday 8th September, Magdalena will be joining the team at MCR Injury Clinic.

She’s coming to us from a highly respected clinic where she’s worked with some genuinely high-level patients, and honestly, getting her on board is a huge upgrade to our clinic team.

The kind of experience she’s bringing in is not something you get every day, and we could not be more excited about what it means for the clinic and for the level of care we can offer.

Told to give up football. 3 months post meniscus surgery.

4 months post grade 2 rec fem tear, both never properly rehabbed.

Semi pro footballer, previous physio unsure where to start, NHS said hang up the boots.

We started with a full capacity test to see exactly where he stood.

4 weeks later, here’s the difference.

Calf raises: 31 up to 40
Sit to stand: 3 up to 10
Bridges: 10 up to 25
Single leg hold: 20 seconds up to 40 seconds

Strength testing on our force plates told the same story.

Knee extension asymmetry down from 11.6% to 8.5%.
Hip flexion asymmetry down from 29.5% to 2.7%.

Numbers don’t lie, and neither does hard work.

Nowhere near done yet, but this is what rebuilding a career looks like one test at a time.

Quick tip on reintroducing early plyometric exercises into your workouts.

Particularly useful if you have never done this type of movement. Or you have had a drop in expose to this type of moment due to injury or time of training.

Most patients plateau on calf raises because they keep adding reps and weight and not changing the stimulus.

Here’s a simple fix: elevate the forefoot on a wedge before you raise. And bend the knee.

This increases ankle dorsiflexion range, placing more stretch load on the soleus and deep calf muscles.

The muscles a flat-floor calf raise barely touches…

Same exercise. Completely different demand.

Small variation, drives big adaptation.
