About the Role:

As a Health Care Assistant, you will provide person-centered care and support to individuals in their own homes, helping them maintain their independence, dignity, and quality of life.

You will support a variety of individuals, including older adults, people living with dementia, physical disabilities, long-term health conditions, and those requiring rehabilitation or end-of-life support.

No two days are the same. You will build meaningful relationships with service users while providing practical support, companionship, and reassurance that enables them to remain safe and comfortable within their own homes.

Main Duties and Responsibilities:

Personal Care and Support
Assisting with washing, bathing, showering, dressing, grooming, and personal hygiene.
Supporting individuals with continence care while maintaining dignity and privacy.
Assisting with mobility, transfers, and repositioning using appropriate equipment and safe moving and handling techniques.
Supporting service users with eating and drinking whilst promoting independence.

Medication Support
Assisting with medication administration, prompting, and monitoring in accordance with care plans and company policies.
Accurately recording medication support provided.
Reporting concerns, side effects, or changes in health promptly.

Nutrition and Hydration
Preparing meals, snacks, and drinks according to individual preferences and dietary requirements.
Encouraging adequate nutrition and fluid intake.
Monitoring and reporting concerns regarding food and fluid consumption.

Emotional and Social Support
Providing companionship and emotional reassurance.
Encouraging participation in hobbies, interests, and community activities.
Supporting individuals to maintain relationships with family and friends.
Promoting independence, confidence, well-being, and choice.

Record Keeping and Communication

Maintaining accurate electronic care records.
Completing daily care notes, medication records, and incident reports.
Reporting changes in health, well-being, behavior, or circumstances.
Communicating effectively with colleagues, healthcare professionals, service users, and family members.

Health, Safety, and Safeguarding

Following care plans, risk assessments, and company procedures.
Identifying and reporting safeguarding concerns immediately.
Following infection prevention and control procedures, including the use of PPE.
Promoting dignity, respect, equality, and person-centered care at all times.

Essential Requirements

Right to work in the UK.
Good spoken and written English.
Willingness to undergo an Enhanced DBS check.
Ability to work flexible hours, including weekends and bank holidays where required.
Commitment to completing mandatory training and ongoing professional development.

Desirable Requirements

Previous experience within health and social care.
NVQ/QCF Level 2 or Level 3 in Health and Social Care.
Full UK Driving License and access to a vehicle.
Experience supporting individuals with dementia, mental health conditions, learning disabilities, or complex care needs.
