We have recently invested in a system called Abtrace
This system allows us to see absolutely everything that is outstanding for any one patient at a time
We can have your medical record open on one screen and Abtrace on the other
This will hopefully allow us to address everything that is due or overdue from
Blood tests to smears to vaccines and much more β¦ meaning less trips to the practice for our patients
We can see
πΉ the conditions you have
πΉ The investigations that are needed for your condition and the medication you are taking
Everything is colour coded
π’ Green have been dealt with
π Amber are approaching their due date
π΄ Red are overdue
πΉThe status of your medication - whether it has been reviewed in the last 12 months
πΉ Any diaries that have been set up for additional reviews ( scans etc )
If you have a long term condition such a Diabetes, Heart Disease, Kidney Disease, Asthma etc we can send a message to you as usual on the month before your birth month but we can also include a booking link
We have programmed it to know which clinician you need to see and whether you need a blood test or not
When you click on the link it will take you straight through to our appointment system
You can click the link and book an appointment at a time convenient for you ( great if you work or have other commitments )
It is available 24/7 as it is an automated service
It saves you having to call us to book and will reduce call volume β¦ allowing our reception team to deal with the increasing number of calls we are receiving and be able to answer the calls that require our help / support
( waiting time on the phones should reduce as we receive several hundred calls for Long Term Condition Review appointments as a lot should be saved if patients use the booking links )
If you have asthma or COPD and no other condition, your booking link will take you straight to the nurses that deal with the respiratory conditions
If you have a condition that requires a blood test before your review, the booking link will direct you to the blood test appointments with Andrea and then Jenny
Andrea and Jenny will use Abtrace and take your blood for anything that is due or overdue and will then look at who you need to see next
They will give you a large laminated card to give to reception ( we are making them EXTRA LARGE so it will be difficult for you to leave without booking your next appointment ( too big for your pocket or handbag)
If you need to bring a sample of urine with you to your next appointment, they will also give you a sample pot which you must bring with you to the next appointment
Just simply take the card straight to reception and book your review appointment
When you give it to reception, this will inform them of who you need to see and how long you need for your next appointment
This appointment is routine and will be 1- 2 weeks later. It will allow the clinician to have access to your results as part of your review
Once the review is complete, if everything is okay then we will be in contact again the following year however if you need support with anything inbetween your annual review appointments, just give us a call
If the nurse feels you need a bit of support to get your results to where they need to be, then they will arrange interim review appointments throughout the year to work with you to get your results to a satisfactory level in order to manage your condition
Before Abtrace β¦.. we would deal with everything that appeared to be due at the time β¦. but sometimes there may be tests that flag up AFTER you have seen seen and with multiple areas to check, it wasnβt always clear
We sometime ended up asking patients to come in again - which isnβt ideal ( for you or us )
Everything is now on one screen, is colour coded and we can see everything that is coming up ππΌ
We will seek patient feedback when it has been up and running for a while
We are hoping the booking links are useful and easy to use
