🩺📄 Fit Notes — formerly known as “Sick Notes” 📄🩺
BMG patients, here’s some helpful information if you are off work due to illness 👇
💡 For the FIRST 7 days of sickness, you can self-certify yourself. You do NOT need a fit note from the GP surgery during this time.
📞💻🏥 If you are still unwell AFTER 7 days, please contact us to request a fit note by:
✔ Online request
✔ Telephone
✔ In person
Your request will be reviewed by a clinician, so please remember this is NOT an instant service , but can be backdated to the point of first contact.⏳ You will also need to be assessed by a clinician, which can be in a face to face appointment or by phone .
👉 Please factor this in and always keep your employer updated with what is happening. Communication with your employer is really important.
🔄 Need to renew an existing fit note?
Please submit your request no more than 3 days BEFORE your current fit note expires, earlier submissions will unfortunately not be processed. This helps avoid delays with the processing and gives time for your request to be reviewed by our clinicians.
Again, please keep your employer informed that:
📌 You are not yet fit to return to work
📌 You have requested an extension from the surgery
📌Fit notes can be back dated not post dated
✅ Good news — if you feel well enough to return to work before your fit note ends, you do NOT need a “back to work” note from us. Simply return to work with your employer’s knowledge. 💙
