* Work Closely with GPs and Practice Teams.
* Work with Patients and their Carer to support in understanding their needs.
* Work alongside the Social Prescribing Team to provide and all encompassing approach to Personalised Care.
* Support and advise on NON medical needs.
* Support people 16+
