Meet the people behind The Castle Clinic. ❤️

We’re Lisa and Justin Dinley, and we founded The Castle Clinic with a simple belief: we wanted to practise dentistry and aesthetic medicine the way we believe they should be practised.

That meant creating a practice where we could make decisions based on what’s genuinely best for our patients, without compromise.

Where we could invest in the equipment we believe in, choose the materials we trust, and create an environment where patients feel comfortable, valued and cared for.

But just as importantly, we wanted to surround ourselves with people who share those same values.

Today, we’re incredibly proud of the team we’ve built. Every member of The Castle Clinic plays a part in upholding the standards and philosophy that inspired us to open our doors.

For us, it’s never been about doing the most treatment. It’s about doing the right treatment, for the right reasons, with our patients’ best interests at heart.

That’s what The Castle Clinic was built on.

When should you refer a patient to a Specialist Endodontist? 🤔

The most useful time to ask for support is often before treatment begins. Retreatment, difficult anatomy, calcified canals and teeth supporting complex restorations all deserve careful case assessment. 🧐

Persistent disease after root canal treatment may reflect an untreated canal, but we also need to consider the coronal seal, remaining infection, cracks and the wider diagnosis.

In this practitioner education carousel, Dr Justin Dinley explains the situations that should prompt an early specialist discussion. The decision depends on the toothache the patients and your own experience and equipment.

When the case becomes difficult.

There is no single referral threshold for every dentist. Compare case difficulty with your training, experience and resources. Patient factors such as limited opening, isolation and diagnostic uncertainty matter as well as the radiograph. Explain the reason for referral and options to the patient.

Retreatment starts with diagnosis and restorability. Review the clinical findings, original treatment date and serial radiographs. A residual radio licence can represent ongoing healing, so one image alone does not prove failure. Discuss retreatment, surgery, monitoring or extraction according to the findings and patient preferences.

Missed anatomy is one possible cause of persistent disease. Think about MB2 canals, additional medial canals, extra canals in incisors and premolars and canals that divide or merge. There is something particularly rewarding about helping a patient keep a tooth they thought they might lose. 🦷

Extra oral drainage associated with infected lower incisors ( 41/31/32) that required Re-RCT. The 42 was immediately responsive to endoice and EPT; this will be monitored.
Many dentists worry about the MB2 in first and second maxillary molars. My belief is always expect one and if you can’t find it be worried or ideally get a CBCT first, if the axial cross section is not round expect 2 MB canals not 1 ( if you struggle; book Daniel Flynn MB2 course)

The same for central and lateral incisors, I often find that these teeth are regarded as easy to treat .If the tooth is deep M-L ( ie not more round), a vertical line Mesially or Distally when taking a parallax view or the canal is NOT centred ( on a CBCT) expect 2 canals, upto 40% of lower anteriors have 2 canals.
This case was exceptionally tricky as lingual canals were not visible and had to be accessed through a round not oval canal or***ce as the split was mid root and not more coronal, all shaping files were pre- curved after initial scouting and effectively coronal enlargement with COXO reciprocating hand piece from Toothsaver with 10/15 handfuls.
Obturated with prebent single cone GP ( chilled with endoice) and onefil BC sealer.
Removal of GP and preparation was done with Reciproc Minima 20 (QED)

They’re pointing at a tooth.
Perhaps it’s even already been root treated.

Persistent dental and facial pain can be one of the most challenging areas of diagnosis — and sometimes the most important clinical decision we make is not to treat.

Raad is a Specialist Endodontist with postgraduate specialist training from the Eastman Dental Institute. He has a particular interest in complex root canal treatment and helping patients preserve their natural teeth.

Just as importantly, Raad brings a calm, thoughtful approach to care. He takes time to listen and has experience supporting patients who feel nervous about treatment.
