a time to celebrate kindness, inclusion, and the many different people and families in our community. Pride Month reminds us that everyone deserves to feel safe, respected, and proud of who they are.
For children, Pride can be a wonderful opportunity to learn about:

As CCNT Transition Nurse, my role is to support young people and families through this important stage, helping make the move from children’s to adult services feel less overwhelming.

I can provide guidance, answer questions, help coordinate services, and offer support tailored to each young person’s needs throughout the transition journey.

We also run a monthly Transition Clinic at ICC for patients under CCNT.

As a child with a life-limiting or life-threatening condition enters their teens and early adulthood, they will move from children’s services to adult services. This process is called transition. With any change comes uncertainty. So it can naturally be a very worrying time for parents, carers and young people.

Simply knowing what support you can expect and how adult services are different from children's can help.
