People often ask which is better: coming to clinic, or having a physio come to you.

Honest answer: it depends on the goal.

Clinic gives you the equipment, the space and the hydrotherapy pool. Home gives you the real stairs, the real bathroom, the real kitchen, the actual doorway you keep catching your elbow on.

Most people we work with end up doing both, and the balance shifts as things change.

You do not have to choose on day one. That is what the assessment is for.

"Improve mobility" is not a goal. It is a wish.

"Stand at the kitchen counter long enough to make a cup of tea without holding on" is a goal. You know when you have got there. So does everyone around you.

Good rehab goals are specific, they matter to you, and they are yours rather than ours.

What would you put on your list? Getting up the stairs. Carrying a mug without spilling it. Walking to the end of the road. Holding a grandchild.

Three things you are entitled to expect from rehabilitation. Not just our opinion, this is national guidance.

1. A proper assessment of the whole picture, as soon as possible. Not just the most obvious problem.

2. Goals agreed with you and written down. Agreed with you, not decided for you.

3. One named person you can actually contact when you do not understand what is happening.

That third one is the one most people do not have.

So here is a question worth asking: if someone rang you today and said ""who is your single point of contact?"", could you name them?

If not, it is entirely reasonable to ask.

Everyone asks how they are doing.

Almost nobody asks how you are doing.

If you are the one lifting, driving, chasing appointments, arguing with the system and still trying to be a wife, husband, daughter or son rather than a support worker, this one is for you.

You are allowed to ask for help before you are at breaking point.

You can contact us on behalf of someone else. You do not need a referral, and you do not need to have it all worked out first.

Meet Jacob Youngson, Senior Physiotherapist at Healthsec Rehab.

Jacob is usually the first clinician a referrer speaks to, and often the one who carries out the initial assessment. What he is not, is a lone practitioner.

Behind every case sits our wider physiotherapy and MDT network, drawn in as the case requires it: neuro-specialist physiotherapy, psychological therapy, wellbeing coaching, hydrotherapy and adjunct interventions. We build the team around the client, rather than fitting the client into whichever team happens to be free.

Two things hold that together:

Equal access. Every enquiry gets the same route in, the same standard of assessment, and the same clinical governance behind it.

Individual plans. Nobody gets a package off a shelf. Goals are set with the client, reviewed on a schedule, and changed when life changes.

Same team, whether the session happens in clinic, at home, or somewhere in the community.

The hardest day is often the first Monday at home.

The ward was busy. There were therapists, a routine, someone checking in. Then the front door closes and it is just you, the person you love, and a folded sheet of exercises on the kitchen table.

If you are in that week right now: you are not doing it wrong. Nobody hands you a manual for this bit.

Rehab does not have to stop at the hospital doors. We work with people at home, in the community and in clinic, at whatever pace makes sense.

Five things people say in their first appointment.

"I've probably left it too long." Almost nobody has. We regularly work with people two and three years down the line who make real progress.

"I don't want to make a fuss." You are not making a fuss. You are asking a question. It is literally our job.

"It's just old age, isn't it." Some of it might be. A surprising amount of it is strength, balance and confidence, and all three respond to training.

"I should be further on than this by now." Says who? Recovery is not a timetable, and comparing yourself to someone else's is a fast route to feeling terrible.

"My husband made me come here." Fair enough. Very often it is the person watching who rings first, and that is frequently how someone finally gets the help they needed.

If you recognised yourself in one of those, that is your sign.

I don't really know what to ask for.

That is how most calls to us start, and it is a completely fine place to begin.

Here is what actually happens when you ring.

You speak to a person (Usually Jacob), not a menu. You describe what is going on — or what is going on with the person you are worried about. You can talk through the whole story, if it helps. Most people do.

We ask some questions and tell you honestly whether we think we can help. Sometimes the answer is that someone else would be better, and we will say so.

If we can help, we arrange an assessment. Clinic, your house, or somewhere in the community. Whichever is easier for you.

Then we agree a plan together, and get going.

No referral needed. No forms to fill in first. And you are allowed to ring on behalf of someone who would never ring themselves.

A quick introduction, in case you are new here.

We are Healthsec Rehab, based at Hadnall just outside Shrewsbury. We provide physiotherapy and rehabilitation for people whose recovery is not finished.

Mostly that means people who have come out of hospital, after a stroke, a brain injury, a spinal injury, an operation or a bad accident. For those who have found that the therapy stopped a long way before they felt ready.

We work in clinic, in people's homes, and out in the community, because the stairs you actually need to manage are the ones in your own house.

You do not need a referral. You can ring us about someone else. And you do not need to have anything figured out before you get in touch.

We are going to be posting a lot more practical things for families and carers here over the next few months, not just adverts.
