Two years in the making. One shared vision. And today, it finally felt real. 💙

Today we hosted our first network meeting at Eastwood Community Hub, bringing together many of the teams and services who will be working together as part of our new 🌟Health & Justice Partnership🌟

From next week, Probation will be based at the Hub every fortnight on Thursday afternoons, alongside a Health Connector with lived experience of the justice system and a PCN volunteer, helping people navigate and connect with the support around them.

In the room today were colleagues from the South Nottinghamshire Probation team, Change Grow Live (CGL), PCN mental health occupational therapy, PCN care coordination, Broxtowe Borough Council and Adult Social Care, with social prescribing, Sexual Health, Citizens Advice, Housing, CVS, Employment Support and many other services also part of this partnership.

The passion in the room was palpable.

Different organisations

Different expertise

But a shared vision and determination to provide support that truly makes a difference for people navigating multiple complex systems.

There was a real sense of “together we can achieve more!” and after two years of bringing this vision to life, we’re so excited that next week, it all begins. 🌱

💪 Our Stronger with Parkinson’s group is growing! 💪

We’ve been so pleased with the response to our Stronger with Parkinson’s sessions that we’re delighted to announce that another group is being added! 🎉‼️📣🎉

Delivered by the wonderful L Leisure, these small-group sessions use specialist exercise equipment designed to support people living with Parkinson’s to build strength, improve movement and stay active, all in a friendly, supportive environment🌟♥️🌟

☕ And it’s not just about exercise… there’s also the opportunity to have a coffee and a chat afterwards, with partners and carers welcome too.

Not sure whether the equipment or sessions would be right for you?
That’s absolutely fine. You can arrange a visit to see the equipment and have a go on the machines before deciding whether the group is suitable for you.

📍 Sessions take place every Friday at Stapleford Pavilion.

We are UTTERLY THRILLED to share that our Nottingham West PCN team has won the PrescQIPP National Award for:

💊 Data Tools and Technology to Support Medicines Optimisation

for their brilliant project:

“Smarter DOAC Management – Optimising Safe Prescribing and Clinician Time through Technology”

…and then, as if winning the national category wasn’t enough… they went on to win the OVERALL GOLD AWARD! 🥇🏆

We genuinely couldn’t be prouder.

Behind this award is a team who saw a problem, came up with a great idea, used technology creatively, and then made it happen, improving safety and care for patients while also making better use of precious clinician time.

But what makes us especially proud is the people behind this project. 💙

We LOVE seeing our staff have the confidence to generate ideas, the determination to see them through, and the creativity to turn those ideas into real innovation that makes a difference to patients.

This is exactly what we want to see at Nottingham West PCN: great people, great ideas, better care AND teams who are genuinely enthused and engaged in making things better.

We’re really proud to share our 2025–26 Annual Review, a chance to look back at what we’ve achieved together across Nottingham West over the past year.

💜What a great turnout for our Domestic Abuse Awareness Training today 💜

44 non-clinical colleagues from across our Beeston & Chilwell practices came together for an important and thought-provoking session delivered by the fabulous Chris from Broxtowe Women's Project Ltd (BWP) with Clare, our PCN Clinical Lead for Health Inequalities, supporting the session.

The training was arranged following learning from a local domestic abuse-related death review, providing an opportunity for our primary care teams to strengthen awareness, share learning and consider how we can continue to support people experiencing domestic abuse.

A huge thank you to Chris for sharing her expertise, to all our colleagues who made the time to take part, and to Broxtowe Borough Council for kindly hosting us.

This is the first of our planned sessions across the borough, with training for our clinical teams to follow, continuing our commitment across the PCN to making sure that health inequalities and safeguarding are not just things we talk about, but something we actively address in our everyday practice. 💜
