Most breast changes won’t be cancer—but it’s always important to get them checked.

Know what’s normal for you and look out for:
• New lumps or swelling
• Changes in size or shape
• Skin changes or dimpling
• Ni**le changes or discharge

In this role, you’ll work directly with GP practices across Sutton, supporting patients with mental health needs, improving pathways between primary and secondary care, and helping shape a more joined‑up, community‑focused service.

💼 What you’ll do:
• Provide mental health triage, assessment and support in GP practices
• Offer step‑down sessions for patients leaving secondary care
• Support patients transitioning into secondary care when needed
• Work closely with GPs, NHS Trust colleagues and community partners
• Help improve mental health outcomes across Sutton
