One of the most frustrating things about weight gain is that there often isn't one simple cause.

Sometimes it's food.

Sometimes it's less movement because of pain or injury.

Sometimes it's poor sleep, stress or hormonal changes.

And quite often it's a combination of several things.

That's one of the reasons we've built a multidisciplinary clinic.

One of the most frustrating things about midlife can be feeling as though your body has changed even though your habits haven't.

Your clothes fit differently.

Weight seems easier to gain.

And the things that worked 10 years ago don't seem quite as effective now.

The good news is that there is usually a reason.

The less good news is that it's rarely just one thing.

One of the things our GPs spend a lot of time helping people with isn't diagnosing rare conditions.

It's helping people work out where to start.

Symptoms like tiredness, low energy, brain fog and feeling "not quite right" can have lots of possible causes.

Sometimes a conversation is the most useful first step.

Our new patient GP appointments are 30 minutes long, giving you time to discuss what's been going on and decide what, if any, investigations might be helpful.

One of the mistakes people often make is assuming tiredness must either be:

A) something serious

or

B) "just life".

The reality is usually somewhere in the middle.

Poor sleep, stress, busy lives, diet, hormones, low iron, thyroid issues and vitamin deficiencies can all play a role.

The tricky bit is working out what's relevant for you.

Getting back on track doesn't always require a complete overhaul.

One of the biggest mistakes people make when their routine gets disrupted is assuming they've failed.

A busy week.

More blood tests do not always mean better healthcare.

In fact, large numbers of unnecessary tests can sometimes:

* create anxiety
* produce confusing or borderline results
* and lead people further away from understanding what’s actually going on

Very often, a smaller number of targeted tests guided by symptoms, history and clinical judgement is far more useful.

That’s one of the reasons we offer:

* GP appointments
* clinician-guided blood testing
* and structured health screening packages

The aim isn’t to do every test possible.

It’s to do the right tests, at the right time, for the right reasons.

One of the things we’re seeing more and more in clinic is people contacting us with lists of blood tests suggested by ChatGPT or other AI tools.

And honestly? Sometimes they’re useful.

AI can be a great starting point for thinking about symptoms and possible investigations.

But one of the difficulties is that the same symptoms can lead to wildly different suggestions depending on how questions are asked - and sometimes people end up considering very large or expensive panels of tests that may not actually be the most useful place to start.

Very often, the difficult part isn’t accessing blood tests.

It’s understanding:

* which tests are actually appropriate
* which are most useful to start with
* and what to do with the results afterwards

One of the things we hear most often from people considering ADHD assessment is:

“I’m not even sure if this is the right step.”

And honestly, that’s completely understandable.

Deciding whether to pursue assessment for yourself or your child can feel like quite a big decision, especially when there’s so much information online and many people don’t feel certain about what they’re experiencing.

That’s one of the reasons we offer free suitability calls and pre-screening questionnaires at Total Health West Berkshire.

Miriam, our assessment coordinator and one of our ADHD assessors, spends a lot of time helping people talk through their concerns, answer practical questions, and decide whether assessment feels appropriate for them - in a calm, supportive and no-pressure way.

Many people questioning ADHD are not obviously hyperactive or chaotic.

In fact, many are highly capable, hardworking people who have spent years developing ways to cope, compensate, and push through difficulties that other people may never fully see.

For some people, it’s only later in life - during parenthood, burnout, increasing work pressures or menopause - that they begin recognising patterns and wondering whether ADHD may have always been part of the picture.

The same is often true for children too. ADHD doesn’t always look the way people expect it to.

I’ve written more this week about some of the ways adults and parents start wondering whether assessment may be worth considering.
