The Hyperoxic-Hypoxic Paradox: The Fascinating Science Behind HBOT

Hyperbaric Oxygen Therapy is about more than simply breathing oxygen.

Inside a hyperbaric chamber, increased atmospheric pressure allows significantly more oxygen to dissolve into the blood plasma, increasing oxygen availability throughout the body.

But what happens after the session is just as fascinating.

As oxygen levels return towards normal, the relative change in oxygen availability can activate cellular signalling pathways normally associated with low oxygen. This phenomenon is known as the Hyperoxic-Hypoxic Paradox.

Surgery is just the first step. How you heal matters.

After cosmetic or reconstructive surgery, your body needs oxygen to repair damaged tissue, form new blood vessels and support the healing process.

Hyperbaric Oxygen Therapy (HBOT) significantly increases the amount of oxygen available to the tissues, supporting the body’s natural repair processes during recovery.

HBOT can support:

A 2025 systematic review and meta-analysis examined 11 studies involving 734 aesthetic-surgery patients. The researchers concluded that HBOT was an effective adjunct in aesthetic surgery, reporting faster recovery and high levels of patient satisfaction.

Another study involving 356 tummy-tuck patients found postoperative complications were significantly lower in those receiving pre-operative HBOT — 8.4% compared with 32.6% in the group that did not receive HBOT.

HBOT has been investigated around procedures including facelifts, tummy tucks, breast surgery, rhinoplasty, liposuction, skin grafts and reconstructive surgery.

By delivering oxygen under increased pressure, HBOT allows substantially more oxygen to dissolve into the blood plasma and reach tissues where oxygen demand can be particularly high following surgery.

HBOT & YOUR BODY’S OWN REPAIR CELLS

One of the most interesting areas of HBOT research is its effect on the body’s own stem and progenitor cells.

A human study looking at Hyperbaric Oxygen Therapy at 2.0 ATA found that circulating CD34+ cells approximately doubled after a single HBOT session.

After a course of 20 sessions, researchers reported an approximately 8-fold increase.

These cells are involved in the body’s natural repair and regeneration processes, and the researchers found that HBOT stimulated their release from the bone marrow into the circulation.

It’s a great example of how HBOT isn’t simply about delivering more oxygen during a session. Exposure to oxygen under pressure can trigger biological responses that continue beyond the time spent inside the chamber.

Is air-only hyperbaric therapy beneficial?

The answer is yes — and the research behind it is very interesting.

When most people think about Hyperbaric Oxygen Therapy (HBOT), they naturally assume the benefits are all about breathing higher concentrations of oxygen.

However, emerging research suggests that pressure itself may also produce important biological effects, even when breathing normal air without supplemental oxygen.

One interesting study investigating mild hyperbaric pressure with normal air reported approximately a 2.5–3x increase in circulating stem/progenitor cell mobilisation.

So what could be happening?

One hypothesis is that increased pressure and nitrogen exposure may influence reactive nitrogen species (RNS), alongside the reactive oxygen species (ROS) more commonly associated with oxygen-based hyperbaric therapy.

At controlled levels, these molecules act as important signalling mechanisms within the body and are involved in cellular adaptation, vascular signalling and tissue responses.

This could help explain why pressure alone appears capable of producing measurable biological effects.

Air-only hyperbaric exposure isn’t the same as breathing high concentrations of oxygen under pressure, and considerably more research is needed to fully understand the differences and potential applications.

But the research highlights something very interesting:

Hyperbaric therapy may not just be about oxygen. Pressure matters too.
